姚靖运用养阴清热法治疗睑板腺功能障碍型干眼经验

摘　　要：姚靖教授指出肺、脾两脏功能失调是导致睑板腺功能障碍型(MGD)干眼发病的主要病因,由此创立“养阴清热法”用于治疗MGD干眼。养阴清热法以养阴润肺生津、清热健脾祛湿为主要治疗原则,治疗方法包括清眩润目饮内服及外用熏目、针刺治疗、太阳穴刺络放血拔罐治疗。养阴清热法能够有效改善MGD干眼患者眼部症状及全身症候。本文将从病因病机、方药组成、针刺治疗、验案举隅等方面对姚靖教授采用养阴清热法的中医理论内涵及临床应用进行介绍,以期能够指导临床。Professor Yao Jing points out that the dysfunction of lung and spleen is the main cause of dry eye with meibomian gland dysfunction.Therefore,the method of nourishing yin and clearing heat is established to treat dry eye with meibomian gland dysfunction.The method of nourishing yin and clearing heat takes nourishing yin,moistening lung and engendering fluid,clearing heat,strengthening spleen and removing dampness as the main treatment principle.The treatment methods include oral administration of Qingxuan Runmu Decoction and external fumigation,acupuncture,cupping after bleeding in the temple area.The method of nourishing yin and clearing heat can effectively improve the ocular and systemic symptoms of dry eye patients with meibomian gland dysfunction.This paper introduces the TCM theoretical basis and clinical application of the method of nourishing yin and clearing heat from the aspects of etiology and pathogenesis,prescription composition,acupuncture treatment and case examples and expects to provide references for clinical practice.
