摘　　要：目的:了解甲状腺癌手术患者住院费用构成以及影响因素,为降低甲状腺癌手术患者住院费用提供依据。方法:选取宜昌市某三级甲等医院2015-2019年2014例甲状腺癌手术患者,利用灰色关联分析法和结构变动度对其住院各项费用的关联度、结构占比及变动情况进行研究。结果:2015-2019年,检查费的关联度最大,其次是耗材,对次均住院费用结构变动贡献率排名前三位的依次为药品费、耗材费和其他费用,三者累计结构变动贡献率为65.30%。药品费、耗材费、检查和未归类费用是影响次均住院费用的主要项目。结论:药品和耗材控费成效显著,建议通过加强诊疗规范、合理检查以及继续控制药品耗材费用等方法,减轻甲状腺癌手术患者住院医疗负担。Objective:To study the composition of average hospitalization expense and the major influential factors of the expenses among thyroid cancer patients with operation,in order to provide evidence for reducing the hospitalization expense.Methods:2014 thyroid cancer patients with operation between 2015 and 2019 were selected from a tertiary level hospital in Yichang.The gray correlation analysis was used to analyze the correlation between different categories of costs with the total expense,and degree of structure variation was applied to explore the changes of internal compositions.Results:From 2015 to 2019,in gray correlation a-nalysis,the examination fees and medical consumable material fees were the main influential factors of medical expenses.The three leading items which affected structural change rate were drug fees,medical consumable material fees and other unclassified fees.The combined structural change rate of these three components contributed 65.30%.Drug,medical consumable material,examination and other unclassified expenses were the main factors which affected the average hospitalization expenses.Conclusion:The costs of medicines and medical consumables have been reduced significantly.Diagnosis and treatment standardization,limitation of the unreasonable use of examinations and rational use of drugs and medical consumable materials should be carried out to relieve economic burden among thyroid cancer patients with operation.
