CD138在宫腹腔镜联合治疗不孕症中的应用研究及相关因素分析

摘　　要：目的探讨CD138在宫腹腔镜联合治疗不孕症中的应用价值。方法2019年1月-2020年6月,选择佛山复星禅诚医院因不孕症行宫腹腔镜联合手术的130例患者作为观察对象,分为慢性子宫内膜炎(CE)组和非CE组。比较两组之间分娩史、异位妊娠史、宫腔手术史、宫颈支原体感染史、流产史、输卵管阻塞、IUD史、EMT史的统计学差异;比较两组患者治疗后临床疗效的统计学差异。结果CE组和非CE组之间,分娩史、异位妊娠史、宫腔手术史、宫颈支原体感染史比较,无显著差异(P>0.05);流产史,输卵管阻塞、IUD史、EMT史比较,有显著差异(P<0.05),以流产史、输卵管阻塞,IUD史、EMT史为自变量带入Logistic回归方程,可知IUD史、输卵管阻塞、流产史以及EMT史的OR值分别为1.632、1.209、0.845、0.571,均显示为CE发生的危险因素。结论CD138在宫腹腔镜联合治疗不孕症中可以提高CE的诊断率。IUD史、输卵管阻塞、流产史以及EMT史是CE发生的危险因素,应建议患者进行CD138免疫组化检测。0bjective To explore the application value of CD138 in hysteroscopy combined with laparoscopy in the treatment of infertility.Methods From January 2019 to June 2020,130 patients with infertility undergoing combined hysteroscopic and laparoscopic surgery in hospital were observed and studied.The statistical differences of delivery history,ectopic pregnancy history,uterine cavity operation history,Cervical Mycoplasma infection history,abortion hitoylallopian tube obstruetion,IUD history and EMT history between the chronic endometritis(CE)group and non CE group were compared.There was statistical dfference between the tvo groups.Results There was no significant difference in the history of delivery,ectopic pregnancy,uterine cavity operation and Cervical Mycoplasma infection between CE group and non CE group(P>0.05);there was significant difference in the history of abortion,fallopian tube obstruction.IUD and EMT(P<0.05)Or values of EMT history were 1.632,1.209,0.845 and 0.571,respectively,which showed that CE was a risk factor.Conclusion CD138 can improve the diagnostic rate of CE in infertility treated by hysteroscopy combined with laparoscopy.IUD history,tubal obstruction history,abortion history and EMT history are risk factors of CE,and CD138 immunohistochemical detection should be recommended.
