摘 要:目的探讨血清神经元特异性烯醇化酶(NSE)、胃泌素释放肽前体(ProGRP)、血清淀粉样蛋白A(SAA)在肺癌患者中的表达及与患者临床病理特征的关系。方法采用回顾性方法,收集2019年1月到2020年12月该院收治的肺癌患者108例作为肺癌组、肺部良性病变患者100例作为良性组,收集同期在体检中心进行健康体检的100例体检健康者作为对照组。比较3组血清NSE、ProGRP、SAA水平,并分析血清NSE、ProGRP、SAA水平与肺癌患者临床病理特征的关系。结果肺癌患者血清NSE、ProGRP、SAA水平明显高于良性组和对照组,差异均有统计学意义(P<0.05)。血清NSE、ProGRP、SAA三者联合检测肺癌的灵敏度、特异度分别为90.0%、84.6%。在有吸烟史、肿瘤最大径≥5 cm、TNM分期Ⅲ~Ⅳ期、低分化、小细胞肺癌、有淋巴结转移、有远处转移的肺癌患者中血清NSE、ProGRP、SAA水平明显高于无吸烟史、肿瘤最大径<5 cm、TNM分期Ⅰ~Ⅱ期、高分化、非小细胞肺癌、无淋巴结转移、无远处转移的肺癌患者(P<0.05)。结论肺癌患者血清NSE、ProGRP、SAA水平明显升高,而且与患者临床病理特征密切相关,NSE、ProGRP、SAA联合检测有利于肺癌的早期诊断以及病情评估。Objective To investigate the expressions of serum neuron-specific enolase(NSE),pro-gastrin-releasing peptide(ProGRP)and serum amyloid A(SAA)in the patients with lung cancer and their relationship with clinicopathological features.Methods The retrospective method was adopted to collect 108 cases of lung cancer admitted to this hospital from January 2019 to December 2020 as the lung cancer group,100 cases of benign lung lesions as the benign group,and 100 cases undergoing healthy physical examination in the physical examination center as the control group during the same period.The levels of serum NSE,ProGRP and SAA were compared among the three groups,and the relationship between the levels of NSE,ProGRP and SAA with the clinicopathological characteristics in the patients with lung cancer was analyzed.Results The levels of serum NSE,ProGRP and SAA in the patients with lung cancer were significantly higher than those in the benign group and control group,and the differences were statistically significant(P<0.05).The sensitivity and specificity of serum NSE,ProGRP and SAA combined detection for diagnosing lung cancer reached 90.0%and 84.6%respectively.The levels of serum NSE,ProGRP and SAA in the patients with smoking history,tumor diameter≥5 cm,TNM stageⅢ-Ⅳ,low differentiation,small cell lung cancer,lymph node metastasis and distant metastasis were significantly higher than those in the patients with non-smoking history,tumor diameter tumor diameter<5 cm,TNM stageⅠ-Ⅱ,high differentiation,non-small cell lung cancer,non-lymph node metastasis and non-distant metastasis(P<0.05).Conclusion The levels of serum NSE,ProGRP and SAA in the patients with lung cancer are significantly increased,moreover closely related to the clinicopathological characteristics.Their combined detection is beneficial to the early diagnosis and disease condition evaluation of lung cancer.
