减压放血疗法对膝关节骨性关节炎患者关节功能恢复的影响

摘　　要：目的观察对比双氯芬酸钠缓释片与减压放血疗法对膝关节骨性关节炎患者膝关节功能和炎性因子水平的影响,以探讨减压放血疗法治疗膝关节骨性关节炎患者的效果。方法采用随机数字表法将安丘市人民医院2019年5月至2020年12月收治的92例膝关节骨性关节炎患者分为对照组和观察组,每组46例。对照组患者给予双氯芬酸钠缓释片口服治疗,观察组患者给予减压放血治疗,两组患者均治疗21 d。对两组患者临床疗效、膝关节功能恢复情况、治疗前后西安大略和麦克马斯特大学(WOMAC)骨关节炎指数评分及炎性因子水平进行比较。结果治疗后观察组患者临床总有效率为93.48%,高于对照组的65.22%;观察组患者膝关节功能恢复总优良率为86.96%,高于对照组的63.04%;治疗后两组患者日常活动受限、关节疼痛、僵硬评分及WOMAC总评分与治疗前比均下降,且观察组与对照组比降低,同时观察组患者评分减少率高于对照组;治疗后两组患者血清肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、基质金属蛋白酶-3(MMP-3)水平均降低,且观察组低于对照组(均P<0.05)。结论相较于口服双氯芬酸钠缓释片治疗,采用减压放血疗法治疗膝关节骨性关节炎患者,可通过改善膝关节局部微循环,减轻机体炎症反应,更有利于改善患者患部关节活动情况,促进膝关节功能的恢复,疗效更佳。Objective To investigate and compare the effects of Diclofenac sodium sustained-release tablets and decompression and bloodletting therapy on knee joint function and inflammatory factor levels in patients with knee osteoarthritis,in order to explore the effect of decompression and bloodletting therapy on patients with knee osteoarthritis.Methods A total of 92 patients with knee osteoarthritis who were admitted to Anqiu People's Hospital from May 2019 to December 2020 were divided into the control group and the observation group by random number table method,with 46 cases in each group.The patients in the control group were given oral Diclofenac sodium sustained-release tablets,and the patients in the observation group were given decompression and bloodletting therapy,and both groups were given treatment for 21 d.The clinical efficacy,knee joint function recovery,Western Ontario and McMaster University(WOMAC)osteoarthritis index scores and inflammatory factor levels before and after treatment were compared between the two groups.Results After treatment,the total clinical effective rate in the observation group was 93.48%,which was higher than 65.22%in the control group;the total excellent and good rate of knee joint function recovery in the observation group was 86.96%,which was higher than 63.04%in the control group;compared with before treatment,the daily activities limitation,joint pain,stiffness score and WOMAC total score of the two groups decreased after treatment,and the observation group was lower than the control group,and the decrease rate of the scores of the observation group was higher than that of the control group;after treatment,the serum levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β)and matrix metalloproteinase-3(MMP-3)in the two groups decreased,and the observation group was lower than the control group(all P<0.05).Conclusion Compared with oral Diclofenac sodium sustained-release tablets,decompression and bloodletting therapy for patients with knee osteoarthritis can improve l
