子午流注择时皮肤针疗法改善特应性皮炎患者皮肤屏障功能疗效观察

摘　　要：目的比较子午流注择时皮肤针疗法与子午流注针刺组在治疗特应性皮炎的临床疗效差异。方法将60例特应性皮炎患者随机分成子午流注择时皮肤针组(皮肤针组)和子午流注择时针刺组(针刺组),各30例。2组选穴均按照子午流注纳甲法选择申时相应穴位进行开穴,皮肤针组进行皮肤针叩刺疗法,针刺组则进行常规针刺疗法。2组均每日治疗1次,1周为1疗程,连续治疗3个疗程。观察2组患者治疗前和3个疗程治疗后的血清IgE水平及治疗前后各时点SCORAD积分的变化。结果 2组患者治疗3个疗程后血清IgE水平、SCORAD积分,各时点均低于治疗前(P<0.05),且皮肤针组治疗后各时点SCORAD积分均低于针刺组(P<0.05)。结论子午流注择时皮肤针组在改善特应性皮炎患者的皮肤屏障功能方面疗效显著,明显优于子午流注择时针刺组。这可能与皮肤针叩刺具有激发经络脏腑功能,疏泄气机升降,调控气血运行,参与修复皮肤屏障功能有关。Objective To compare the clinical efficacy of the timing demal needle therapy and the timing acupuncture therapy in the treatment of atopic dermatitis based on the midnight-noon ebb-flow point selection theory.Methods A total of sixty patients with atopic dermatitis were randomly divided into the demal needle therapy group and the acupuncture therapy group,with 30 cases in each.Corresponding points of the two groups were selected according to the midnight-noon ebb-flow point selection theory during the period of the day from 3 p.m.to 5 p.m.The dermal needle therapy group received the dermal needle therapy,while the acupuncture therapy group received the conventional acupuncture therapy.Both groups were treated once a day,with one week as a course and for three consecutive courses as a whole.The changes in the serum IgE levels and SCORAD scores in the 2 groups at each time point before and after treatment were observed.Results The serum IgE levels and SCORAD scores of the two groups at each time point after treatment were lower than those before treatment (P<0.05),and the SCORAD scores of the demal needle therapy group were lower than those of the acupuncture therapy group (P<0.05).Conclusion The timing demal needle therapy is significantly better than the acupuncture thrapy in improving the skin barrier function of patients with atopic dermatitis,which may be related to the function of stimulating the meridians and viscera with the dermal needle tapping,regulating the qi and blood movement and participating in the repair of the skin barrier function.
