摘 要:目的探讨保肺定喘汤用于慢性阻塞性肺疾病(COPD)稳定期的临床疗效及其对SP-D、CC-16的影响。方法60例COPD稳定期患者随机分为对照组和观察组,每组各30例,最终纳入研究46例。对照组22例,予以噻托溴铵粉吸入治疗。观察组24例,在噻托溴铵吸入治疗的基础上联合保肺定喘汤口服,连续治疗12周。比较治疗前后两组的mMRC评分、CAT评分、肺功能、外周血氧饱和度、血清SP-D及CC-16水平和诱导痰细胞分类计数(%)及SP-D、CC-16水平。结果治疗12周后,观察组的mMRC评分、中医症状评分、外周血氧饱和度、舒张前一秒率、血SP-D水平、血CC-16水平、痰SP-D水平、痰CC-16水平、痰中性粒细胞计数(%)与对照组比较,差异均有统计学意义(P<0.05)。结论保肺定喘汤可以改善COPD稳定期患者的呼吸困难等临床症状,改善血氧饱和情况,其作用机制可能与调节SP-D、CC-16的分泌相关。Objective To investigate the clinical efficacy of Baofei Dingchuan Decoction for stable chronic obstructive pulmonary disease(COPD)and its effect on SP-D and CC-16.Methods Sixty patients with stable COPD were randomly divided into control group and observation group,with 30 cases in each group,and 46 cases were eventually included in the study.The control group 22 patients were treated with tiotropium powder inhalation.The observation group 24 patients were treated with tiotropium bromide inhalation combined with Baofei Dingchuan Decoction orally for 12 weeks.The mMRC score,CAT score,lung function,peripheral blood oxygen saturation,serum SP-D and CC-16 levels,sputum cell classification count(%)and SP-D and CC-16 levels were compared between the two groups before and after treatment.Results After 12 weeks of treatment,there were statistically significant differences in mMRC score,TCM symptom score,peripheral blood oxygen saturation,one second rate before diastolic.blood SP-D level,blood CC-16 level,sputum SP-D level,sputum CC-16 level and sputum neutrophil count(%)between the observation group and the control group(P<0.05).Conclusion Baofei Dingchuan Decoction can improve clinical symptoms such as dyspnea and blood oxygen saturation in patients with COPD at stable stage,and its mechanism may be related to regulating the secretion of SP-D and CC-16.
