摘　　要：目的探究Storz输尿管软镜下钬激光碎石术(FURS)治疗复杂性上尿路结石的效果。方法抽取2019年5月至2021年1月于商丘市第一人民医院治疗的复杂性上尿路结石患者105例,按随机数字表法分为观察组(52例)和对照组(53例)。对照组采取经皮肾镜取石术(PCNL)进行治疗,观察组采取FURS进行治疗。比较两组围术期相关指标、疗效及术后复发、并发症发生情况。结果两组术中出血量比较差异未见统计学意义(P>0.05)。观察组手术时间、首次下床时间及术后住院时间均短于对照组(P均<0.05)。两组碎石成功率、结石排净率、临床疗效比较差异未见统计学意义(P>0.05)。观察组术后总并发症发生率(3.84%,2/52)、结石复发率(1.92%,1/52)低于对照组(15.09%,8/53;13.21%,7/53),P<0.05。结论FURS治疗复杂性上尿路结石疗效显著,可加快术后恢复进程,降低术后并发症发生率和结石复发率。Objective To investigate the curative effect of holmium laser lithotripsy under Storz flexible ureteroscopy(FURS)on patients with complex upper urinary tract calculi.Methods A total of 105 patients with complex upper urinary tract calculi treated in Shangqiu First People’s Hospital from May 2019 to January 2021 were selected and divided into observation group(52 cases)and control group(53 cases)according to random number table method.Patients in the control group were treated by percutaneous nephrolithotomy(PCNL),and patients in the observation group were treated by FURS.The perioperative indicators,curative effect,postoperative recurrence,postoperative complications,and changes in quality of life before and after operation were compared between the two groups.Results There was no significant difference in intraoperative blood loss between the two groups(P>0.05).The operation time,first ambulation time and postoperative hospital stay of the observation group were significantly shorter than those of the control group(all P<0.05).There was no statistically significant difference in the success rate of lithotripsy,stone clearance rate or clinical effects between the two groups(P>0.05).The total incidence of postoperative complications and stone recurrence rate in the observation group(3.84%,2/52;1.92%,1/52)were significantly lower than those in the control group(15.09%,8/53;13.21%,7/53),P<0.05.Conclusions FURS is markedly effective in the treatment of complex upper urinary tract calculi,which can accelerate postoperative recovery,and reduce the incidence of postoperative complications and stone recurrence rate.
