摘　　要：目的探讨乳果糖序贯微生态制剂对重症急性胰腺炎(SAP)患者的疗效及对其血清指标和生活质量的影响。方法选择武汉同济医院2016年5月至2019年11月收治的85例SAP患者,根据治疗方式的不同分为研究组(42例)及对照组(43例)。研究组患者在常规治疗基础上联合乳果糖序贯微生态制剂治疗。对照组患者采用常规治疗。比较2组患者的临床疗效、血清指标、生活质量以及肠道菌群差异。结果研究组患者有效率高于对照组(90.5%vs 72.1%,χ^(2)=4.699 0,P=0.030 2)。研究组患者炎性相关血清指标(TNF-α、IL-6)水平及肠道菌群(肠杆菌、肠球菌、双歧杆菌、乳杆菌)数量均较对照组明显改善(均P<0.05)。研究组患者生活质量各项目评分均高于对照组(均P<0.05)。结论乳果糖序贯微生态制剂对SAP患者的疗效更为显著,不仅可减轻其炎性反应,同时还可改善患者肠道菌群及预后,值得临床推广应用。Objective To observe the effect of Lactulose combined with microecological agents on serum indicators and life quality in SAP patients. Methods A total of 85 SAP patients treated in our hospital from May 2016 to Nov 2019 were enrolled and divided into observation group(42 cases) or control group(43 cases). The patients in control group were given conventional treatment while those in observation group were given conventional treatment and Lactulose microecological agent. The clinical efficacy, serum indicators, life quality and intestinal flora were compared between groups. Results The efficiency rate in the observation group was higher than that in the control group(90.5% vs 72.1%, χ^(2)=4.699 0, P=0.030 2). The levels of inflammatory serum indexes(TNF-α, IL-6) and intestinal flora(Enterobacter, Enterococcus,Bifidobacteria, Lactobacillus) in the observation group were significantly improved than those in the control group(all P<0.05). The scores of quality of life in the observation group were higher than those in the control group(all P<0.05). Conclusion The Lactulose microecological agent in the treatment of SAP patients has better effect. It can reduce the inflammatory reaction, and improve intestinal flora and prognosis.
