排石汤联合体外物理振动排石治疗肾结石ESWL术后残余碎片效果研究

摘　　要：目的:探讨中药排石汤联合体外物理振动排石(extracorporeal physical vibration lithotripsy, EPVL)治疗肾结石患者体外冲击波碎石(extracorporeal shock wave lithotripsy, ESWL)术后残余碎片的有效性及安全性。方法:选取进行ESWL治疗的210例肾结石患者作为研究对象,随机分为中药治疗组、EPVL治疗组和中药联合EPVL治疗组,每组70例。比较3组患者ESWL后第1、2、4周结石排净率,首次排石时间、结石排净时间、排石不良反应以及并发症等。结果:第1、2、4周EPVL治疗组和中药联合EPVL治疗组结石排净率、首次排石时间、结石排净时间均明显优于中药组(P<0.05);第1、2周中药联合EPVL治疗组结石排净率均高于EPVL治疗组(P<0.05);中药联合EPVL治疗组首次排石时间、结石排净时间均短于EPVL治疗组(P<0.05)。3组患者治疗期间均未发生严重并发症,仅EPVL治疗组出现1例发热;中药联合EPVL治疗组血尿和白细胞尿发生率均低于EPVL治疗组和中药组(P<0.05)。结论:排石汤联合EPVL治疗能提高肾结石患者ESWL术后残石排净率,缩短排石周期,可减少不良反应的发生且未引起严重并发症,具有较高的有效性和安全性。Objective: To investigate the efficacy and safety of Paishi Decoction combined with extracorporeal physical vibration lithotripsy(EPVL) in the treatment of residual fragments after extracorporeal shock wave lithotripsy(ESWL) in patients with renal calculi.Methods: 210 patients with renal calculi treated with ESWL were randomly divided into Chinese materia medica treatment group, EPVL treatment group and Chinese materia medica combined with EPVL treatment group, with 70 cases in each group.The stone removal rate, first stone removal time, stone removal time, adverse reactions and complications of the three groups were compared at 1,2 and 4 weeks after ESWL.Results: At 1,2 and 4 weeks, the stone removal rate, the first stone removal time and the stone removal time of EPVL group and Chinese materia medica combined EPVL group were significantly better than those of Chinese materia medica group(P<0.05);at 1 and 2 weeks, the stone clearance rate of Chinese materia medica combined with EPVL group was higher than that of EPVL group(P<0.05);the first stone removal time and stone removal time in Chinese materia medica combined with EPVL group were shorter than those in EPVL group(P<0.05).No serious complications occurred in the three groups during treatment, only one case of fever occurred in EPVL group;the incidence of hematuria and leukocyturia in Chinese materia medica combined with EPVL group was lower than that in EPVL group and Chinese materia medica group(P<0.05).Conclusion: Paishi Decoction combined with EPVL can improve the residual stone removal rate after ESWL in patients with renal calculi, shorten the stone removal cycle, reduce the incidence of adverse reactions and cause no serious complications.It has high effectiveness and safety.
