摘  要:目的探讨MTHFR基因677C/T和血清同型半胱氨酸(Hcy)水平与非酒精性脂肪性肝病(NAFLD)的相关性。方法纳入249例成人住院患者,参照NAFLD亚洲诊断标准将其分为NAFLD组(131例)和对照组(118例)。采集两组患者的基本资料并检测Hcy水平和MTHFR基因分型,行Hardy-Weinberg遗传平衡分析后对两组基因型分布情况进行比较,采用二元Logistic回归分析影响NAFLD发生的危险因素。结果 Hardy-Weinberg平衡分析结果显示,两组人群基因达到了遗传平衡(P>0.05)。NAFLD组MTHFR基因型TT的占比高于对照组,两组MTHFR基因分布比较,差异有统计学意义(χ;=25.941,P<0.05)。NAFLD组基因型CT、TT的Hcy水平高于对照组(P<0.05)。二元Logistic回归分析结果显示,BMI、WHR、TG、TC、LDL、HOME-IR、MTHFR基因型TT、Hcy均为NAFLD的危险因素(P<0.05)。结论临床上应加强具有TT基因型、高Hcy水平、高体重、胰岛素抵抗、血脂异常人群的监测,以预防NAFLD的发生。Objective To investigate the correlation between MTHFR gene 677 C/T, serum homocysteine(Hcy) level and nonalcoholic fatty liver disease(NAFLD). Methods A total of 249 adult inpatients were included and divided into NAFLD group(131 cases) and control group(118 cases) according to the Asian diagnostic criteria for NAFLD. The basic data of patients in the two groups were collected, Hcy level and MTHFR genotyping were detected, the genotype distribution of the two groups was compared after Hardy-Weinberg genetic balance analysis, and binary Logistic regression was used to analyze the risk factors of NAFLD. Results Hardy-Weinberg balance analysis result showed that the genes of the two groups reached genetic balance(P >0.05). The proportion of MTHFR genotype TT in the NAFLD group was higher than that in the control group, and there was significant difference in MTHFR gene distribution between the two groups(χ;=25.941, P<0.05). The Hcy level in genotype CT and TT of the NAFLD group was higher than that of the control group(P<0.05). The results of binary Logistic regression analysis showed that BMI,WHR, TG, TC, LDL, HOME-IR, MTHFR genotype TT and Hcy were risk factors for NAFLD(P<0.05). Conclusion The monitoring of people with TT genotype, high Hcy level, high body weight, insulin resistance and dyslipidemia should be strengthened to prevent the occurrence of NAFLD.
