摘 要:目的分析获得性低巨核细胞性血小板减少性紫癜(AATP)和再生障碍性贫血(AA)与Epstein-Barr病毒(EBV)感染的相关性。方法选择2015年1月至2020年1月于我院接受治疗的30例AATP患者(AATP组)与30例AA患者(AA组)为研究对象,另选取同期于我院进行体格检查的50例健康体检者为对照组。检测并比较三组研究对象的EBV-IgM阳性率、EBV-IgG阳性率、EBV-DNA阳性率及EBV-DNA拷贝数,同时就AATP与AA患者血小板计数(PLT)与EBV-IgM阳性率、EBV-IgG阳性率和EBV-DNA阳性率的相关性开展Spearman相关性分析。结果 AATP组和AA组的血清EBV-IgM和EBV-IgG阳性率显著高于对照组,差异具有统计学意义(P<0.05);AATP组与AA组的血清EBV-IgM和EBV-IgG阳性率比较,差异不具有统计学意义(P>0.05)。AATP组和AA组的EBV-DNA阳性率和EBV-DNA拷贝数显著高于对照组,差异具有统计学意义(P<0.05);AATP组与AA组的EBV-DNA阳性率和EBV-DNA拷贝数比较,差异不具有统计学意义(P>0.05)。AATP组与AA组的PLT少于对照组(P<0.05)。Spearman相关性分析结果显示,AATP与AA患者的PLT与EBV-IgM阳性率、EBV-IgG阳性率以及EBV-DNA阳性率均呈现显著负相关(P<0.05)。结论 AATP和AA患者的EBV感染率明显高于常规个体,且EBV-IgM和EBV-IgG在疾病诊断中具有一定的临床意义,考虑可以将EBV检测同常规诊疗相结合用于AATP和AA的诊疗中。Objective To analyze the correlation between acquired amegakaryocytic thrombocytopenic purpura(AATP),aplastic anemia(AA) and Epstein-Barr virus(EBV) infection. Methods A total of 30 AATP patients(AATP group) and 30 AA patients(AA group) who received treatment in our hospital from January 2015 to January 2020 were selected as the study objects, and 50 healthy people who underwent physical examination in our hospital at the same time were selected as control group. The positive rates of EBV-IgM, EBV-IgG and EBV-DNA and EBV-DNA copy number of the three groups were detected and compared. At the same time, Spearman correlation analysis was carried out on the correlation between platelet count(PLT) and positive rates of EBV-IgM, EBV-IgG and EBV-DNA in patients with AATP and AA.Results The positive rates of serum EBV-IgM and EBV-IgG in the AATP group and the AA group were significantly higher than those in the control group, and the differences were statistically significant(P <0.05);there were no significant differences in the positive rates of serum EBV-IgM and EBV-IgG between the AATP group and the AA group(P>0.05). The positive rate of EBV-DNA and EBV-DNA copy number in the AATP group and the AA group were significantly higher than those in the control group, and the differences were statistically significant(P<0.05);there were no significant differences in positive rate of EBV-DNA and EBV-DNA copy number between the AATP group and the AA group(P>0.05). The PLT in the AATP group and the AA group was less that in the control group(P<0.05). The Spearman correlation analysis results showed that the PLT of patients with AATP and AA were negatively correlated with the positive rates of EBV-IgM,EBV-IgG, and EBV-DNA(P <0.05). Conclusion The infection rate of EBV in patients with AATP and AA is significantly higher than that of conventional individuals, and EBV-IgM and EBV-IgG have certain clinical significance in the diagnosis of diseases.
