背俞穴埋线联合舒肝解郁胶囊治疗卒中后抑郁的疗效观察

摘　　要：目的观察背俞穴埋线联合舒肝解郁胶囊治疗卒中后抑郁(PSD)的临床疗效。方法将141例PSD患者按照随机数字表法分为中药组(47例)、穴位埋线组(47例)和联合组(47例)。比较3组临床疗效,比较3组治疗前后美国国立卫生研究院卒中量表(NIHSS)评分、日常生活能力量表(ADL)评分、汉密顿抑郁量表(HAMD)评分和抑郁自评量表(SDS)评分的变化,比较3组治疗前后血清5-羟色胺(5-HT)、中枢神经特异蛋白(S100-β)、脑源性神经营养因子(BDNF)的水平变化。结果联合组总有效率为95.7%,高于中药组的74.5%和穴位埋线组的72.3%,差异具有统计学意义(P<0.05)。治疗后,3组NIHSS评分均低于同组治疗前(P<0.05),ADL评分均高于同组治疗前(P<0.05);联合组治疗后NIHSS评分低于中药组和穴位埋线组(P<0.05),ADL评分高于中药组和穴位埋线组(P<0.05)。治疗后,3组HAMD和SDS评分均低于同组治疗前(P<0.05);联合组治疗后HAMD和SDS评分均低于中药组和穴位埋线组(P<0.05)。治疗后,3组血清5-HT和BDNF水平均高于同组治疗前(P<0.05),血清S100-β水平均低于同组治疗前(P<0.05);联合组治疗后血清5-HT和BDNF水平均高于中药组和穴位埋线组(P<0.05),血清S100-β水平均低于中药组和穴位埋线组(P<0.05)。结论背俞穴埋线联合舒肝解郁胶囊治疗PSD可减轻神经功能缺损程度,提高日常生活活动能力,缓解抑郁程度,调节血清5-HT、S100-β和BDNF水平,疗效优于单纯背俞穴埋线和单纯中药治疗。Objective To observe the clinical efficacy of catgut embedding at back-Shu points plus Shu Gan Jie Yu capsules for post-stroke depression(PSD).Method One hundred and forty-one PSD patients were allocated,using a random number table,to a Chinese herbal medicine group(47 cases),a catgut embedding group(47 cases)and a combination group(47 cases).The clinical therapeutic effects were compared between the three groups.The National Institutes of Health stroke scale(NIHSS)scores,the activities of daily living(ADL)scores,the Hamilton depression scale(HAMD)scores and the self-rating depression scale(SDS)scores were compared between the three groups before and after treatment.Serum 5-hydroxytryptamine(5-HT),central nervous system specific protein(S100-β)and brain-derived neurotrophic factor(BDNF)levels were also compared between the three groups before and after treatment.Result The total efficacy rate was 95.7%in the combination group,which was higher than 74.5%in the Chinese herbal medicine group and 72.3%in the catgut embedding group with statistically significant differences(P<0.05).After treatment,the NIHSS score decreased and the ADL score increased in the three groups compared with before(P<0.05).After treatment,the NIHSS score was lower and the ADL score was higher in the combination group than in the Chinese herbal medicine and catgut embedding groups(P<0.05).After treatment,the HAMD score and the SDS score decreased in the three groups compared with before(P<0.05)and were lower in the combination group than in the Chinese herbal medicine and catgut embedding groups(P<0.05).After treatment,serum 5-HT and BDNF levels increased and serum S100-βlevels decreased in the three groups compared with before(P<0.05).After treatment,serum 5-HT and BDNF levels higher and serum S100-βlevels were lower in the combination group than in the Chinese herbal medicine and catgut embedding groups(P<0.05).Conclusion Catgut embedding at back-Shu points plus Shu Gan Jie Yu capsules can reduce neurological deficits,improve the activiti
