摘　　要：目的:研究分析非小细胞肺癌(NSCLC)患者动态增强磁共振成像(DCE-MRI)定量参数与血清增殖细胞核抗原(PCNA)、凋亡抑制蛋白(Survivin)和血管内皮生长因子(VEGF)表达水平的相关性。方法:选取医院收治的137例NSCLC患者纳入观察组,另选同期进行体检的137名健康者纳入健康对照组。采用磁共振扫描仪测定DCE-MRI定量参数容量转移常数(K^(trans))、速率常数(K_(ep))及血管外细胞外间隙容积比(V_(e))值。采用酶联免疫吸附试验(ELISA)法检测两组血清PCNA、Survivin及VEGF水平,对比分析两组K^(trans)、K_(ep)和V_(e)值,以及PCNA、Survivin和VEGF水平。分析观察组患者血清表达水平以及K^(trans)、K_(ep)和V_(e)值与临床病理特征关系。采用皮尔森(Pearson)相关性分析法分析两组血清PCNA、Survivin和VEGF水平与K^(trans)、K_(ep)及V_(e)值的相关性。结果:观察组患者血清PCNA、Survivin及VEGF水平显著高于健康对照组,差异有统计学意义(t=50.025,t=8.724,t=16.330;P<0.05);观察组患者K^(trans)、K_(ep)及V_(e)值显著高于健康对照组,差异有统计学意义(t=8.258,t=7.937,t=14.046;P<0.05)。观察组患者血清PCNA、Survivin及VEGF表达水平中,低分化、TNM分期Ⅲ期及淋巴结转移均显著高于高中分化、TNM分期I~Ⅱ期及无淋巴结转移患者,差异有统计学意义(t_(PCNA)=4.192,t=4.327,t=5.857;t_(Survivin)=3.197,t=3.428,t=5.140;t_(VEGF)=4.915,t=4.724,t=4.485;P<0.05);患者的K^(trans)、K_(ep)及V_(e)值中,低分化、TNMⅢ期及淋巴结转移均显著高于高中分化、TNM I~Ⅱ期及无淋巴结转移患者,差异有统计学意义(t_(K^(trans))=5.159,t=5.514,t=4.841;tK_(ep)=6.043,t=5.776,t=5.690;tV_(e)=5.265,t=6.076,t=6.783;P<0.05)。观察组患者血清PCNA、Survivin表达水平与K^(trans)、K_(ep)值呈正相关(rPCNA=0.435,r=0.402;r_(Survivin)=0.382,r=0.393;P<0.05),VEGF表达水平与K^(trans)、K_(ep)及V_(e)值呈正相关(r=0.503,r=0.405,r=0.316;P<0.05)。结论:NSCLC患者DCE-MRI定量参�Objective:To research and analyze the correlations between quantitative parameters of dynamic contrastenhanced magnetic resonance imaging(DCE-MRI)and the expressions of proliferating cell nuclear antigen(PCNA),apoptotic inhibitor protein(Survivin),vascular endothelial growth factor(VEGF)in serum of patients with non-small cell lung cancer(NSCLC).Methods:137 patients with NSCLC who admitted to hospital were divided into observation group,and 137 healthy people who underwent physical examination in the same period were divided into healthy control group.The magnetic resonance(MR)scanner was adopted to detect transfer constant of volume(K^(trans)),speed constant(K_(ep))and the volume ratio between extravascular and extracellular interval(V_(e))of DCE-MRI quantitative parameters.The enzyme linked immunosorbent assay(ELISA)was adopted to detect the levels of serum PCNA,Survivin and VEGF of two groups,and the K^(trans),K_(ep) and V_(e) of DCE-MRI were compared and analyzed.And the relations of the expression levels of serum,K^(trans),K_(ep) and V_(e) and clinicopathologic features of observation group were analyzed.The correlations of serum levels of PCNA,Survivin and VEGF and the values of K^(trans),K_(ep) and V_(e) of two groups were analyzed by Pearson correlation analysis method.Results:The levels of serum PCNA,Survivin and VEGF of observation group were significantly higher than those of healthy control group(t=50.025,t=8.724,t=16.330,P<0.05),respectively.And the values of K^(trans),K_(ep) and V_(e) of observation group were significantly higher than those of healthy control group(t=8.258,t=7.937,t=14.046,P<0.05).In the observation group,the expression levels of serum PCNA,Survivin and VEGF of minor differentiation,TNM III stage and lymphatic metastasis were significantly higher than those of high-middle differentiation,TNM I-II stage and nonlymphatic metastasis(t_(PCNA)=4.192,t=4.327,t=5.857,t_(Survivin)=3.197,t=3.428,t=5.140,t_(VEGF)=4.915,t=4.724,t=4.485,P<0.05),respectively.The values of K^(trans),K_(ep) and V
