摘　　要：目的:探讨磁共振成像中表观扩散系统(ADC)及各向异性分数(FA)在慢性酒精中毒性脑病(CAE)的诊断和评估中的价值。方法:收集符合CAE的患者80例,按连续饮酒年限分为四组即<10年;10-20年;20-30年;>30年,每组20例。另选20例无饮酒史的年龄区间相同的病例20例作为对照组。对所有病例进行DTI成像,生成ADC图及FA图,测量额叶白质、半卵圆区、胼胝体膝部、压部及小脑5个感兴趣区双侧的ADC值及FA值。采用双侧对称部位测量,然后取平均值的方法确定感兴趣区的ADC值和FA值。进行组间及组内比较,观察ADC值及FA值与饮酒年限的关系。结果:CAE患者的ADC值及FA值较对照组减低,以ADC值减低更明显,差别有统计学意义(P<0.05);且与患者的饮酒年限有明显的相关性,随着饮酒年限的增加,观察组与对照组之间ADC值及FA值差异更趋明显。结论:ADC值及FA值可对CAE进行定量评估,为CAE的早期诊断和治疗提供参考。Objective:To explore the value of ADC and FA in the diagnosis and evaluation of chronic alcoholic encephalopathy(CAE)in MRI.Methods:Eighty patients with CAE were selected and divided into four groups according to the continuous drinking period,namely<10 years;10-20 years;20-30 years;>30 years,20 cases in each group.Another 20 cases with the same age range without drinking history were selected as the control group.DTI imaging was performed in all cases,and an apparent diffusion coefficient(ADC)map and an anisotropic fraction(FA)map were generated to measure the five regions of interest of the frontal white matter,semi-oval area,corpus callosum,inferior and cerebellum.Side ADC value and FA value.The ADC value and FA value of the region of interest are determined by measuring the bilateral symmetric portion and then averaging.The relationship between ADC value,FA value and drinking years was observed Inter-group and intra-group.Results:The ADC value and FA value of CAE patients were lower than those of the control group,especially the ADC value,and the difference was statistically significant(P<0.05).With the increase of drinking years,the difference of ADC and FA between the observation group and the control group is more obvious.Conclusion:ADC value and FA value can be used for quantitative evaluation of CAE,which can provide reference for the early diagnosis and treatment of CAE.
