双环切口上蒂法治疗乳房肥大的临床研究

摘　　要：目的探讨双环切口上蒂法治疗乳房肥大的临床效果。方法2017年6月-2019年10月,采用双环切口上蒂法治疗乳房肥大53例(106侧),乳房下极中央部分腺体制成舌状乳头乳晕复合瓣,V-Y推进乳头乳晕复合瓣至适当位置,乳晕周围行荷包缝合。结果术后随访6~18个月,47例患者对术后乳房形态满意;3例(3侧)出现了皮下脂肪液化,经换药后切口愈合,3例(6侧)切口出现轻度瘢痕增生。结论采用双环切口上蒂法治疗乳房肥大,可以增加乳房的凸度,确保乳头乳晕血运,保留乳头乳晕感觉及乳房的泌乳功能,切口瘢痕不明显,值得临床推广应用。Objective To explore the clinical effect of the application of superior pedicle combined with double ring incision for the treatment of female hypermastia.Methods From June 2017 to October 2019,superior pedicle combined with double ring incision was performed in 53 patients(106 breasts)with breast hypertrophy for the treatment of female hypermastia.The tongue-shaped nippleareola composite flap was made from the central part of the lower pole of the breast,the V-Y advanced the nipple-areola composite flap to the appropriate position,and purse-string suture was performed around the areola.Results Fifty-three cases were followed-up for 6 to 18 months,47 patients were generally satisfied with postoperative breast morphology.Fat liquefaction occurred in 3 patients(3 breasts)and healed after multiple dressing changes.Scar hyperplasia occurred in 3 patients(6 breasts).The patients were generally satisfied with the postoperative outcomes.Conclusion The application of superior pedicle combined with double ring incision for the treatment of female hypermastia offers a great number of advantages,like better breast shape,ensuring the blood supply of nipple areola,reserved the feeling of nipple areola and the lactation function of breast,invisible scar of incision.With the presence of clinical advantages,superior pedicle combined with double ring incision is worth being widely used to treat female hypermastia.
