摘 要:目的:研究促红细胞生成素(EPO)受体在心肌缺血再灌注过程中的表达及功能。方法:对20只健康雄性SD大鼠进行研究,以随机数字表法分作实验组及对照组,每组各10只。两组均建立心肌缺血再灌注模型,其中实验组造模前24h予以5000U/kg的人重组促红细胞生成素(rHuEPO)腹腔注射,对照组则予以1mL的0.9%氯化钠溶液进行腹腔注射。比较两组EPO受体蛋白表达水平、血流动力学指标水平、心肌凋亡指数、心肌酶谱、心肌caspase-3表达水平。结果:实验组再灌注后4h、6h、8h时EPO受体蛋白相对表达量均高于对照组(均P<0.05)。实验组再灌注后10min、20min、40min、60min时的冠脉流出量、左室发展压恢复率均高于对照组,而左室舒张末压均低于对照组(均P<0.05)。实验组再灌注后4h、6h及8h时的心肌凋亡指数均低于对照组(均P<0.05)。实验组大鼠再灌注后20min冠脉流出液的CK、LDH水平分别低于对照组(均P<0.05)。实验组大鼠再灌注后4h、6h及8h时的心肌caspase-3表达水平均低于对照组(均P<0.05)。结论:EPO受体在心肌缺血再灌注过程中存在明显低表达,可能发挥调控血流动力学、心肌凋亡以及心肌酶谱的功能。Objective:To study the expression and function of erythropoietin(EPO)receptor during myocardial ischemia reperfusion.Methods:twenty healthy male SD rats were studied and divided into experimental group and control group by random number table method,with 10 rats in each group.Myocardial ischemia reperfusion model was established in both groups.The experimental group was intraperitoneously injected with 5000U/kg human recombinant erythropoietin(RHUEPO)24 hours before modeling,while the control group was intraperitoneously injected with 1 mL of 0.9%sodium chloride solution.EPO receptor protein expression level,hemodynamic index level,myocardial apoptosis index,myocardial enzyme profile and myocardial caspase-3 expression level were compared between the two groups.Results:The relative expression of EPO receptor protein in the experimental group was higher than that in the control group at 4h,6h and 8h after reperfusion(P<0.05).The recovery rate of coronary outflow and left ventricular development pressure at 10min,20mi,40min and 60min after reperfusion in the experimental group was higher than that in the control group,while the left ventricular end diastolic pressure was lower than that in the control group(P<0.05).The myocardial apoptosis index of the experimental group was lower than that of the control group at 4h,6h and 8h after reperfusion(P<0.05).The CK and LDH levels of coronary outflow fluid in experimental group were lower than those in control group 20 min after reperfusion(P<0.05).The expression level of myocardial Caspase-3 in experimental group was lower than that in control group at 4h,6h and 8h after reperfusion(P<0.05).Conclusion:EPO receptor is significantly underexpressed during myocardial ischemia reperfusion.It may play a role in regulating hemodynamics,myocardial apoptosis and myocardial enzyme profile.
