基于数据挖掘的中医药治疗慢性乙型肝炎肝郁脾虚证患者用药规律研究

摘　　要：目的:统计治疗慢性乙型肝炎(CHB)肝郁脾虚证患者中药使用情况,探索中药配伍、中药与症状关联的规律。方法:根据纳入、排除标准对2018年3月至2019年12月CHB住院患者的病历资料进行整理汇总,采用数据挖掘技术对处方的中药及证候要素频次、关联等进行分析,探索中药之间的关系及与证候要素之间的关系。结果:共有35份中药处方被纳入研究,白芍使用频率最高为85.7%;核心中药处方包括白芍、陈皮、柴胡、鳖甲、枳壳、茯苓、谷芽;置信度最高的二项、三项中药组合分别为白芍-枳壳、白芍-鳖甲,柴胡;CHB肝郁脾虚证患者的核心证候要素为胁肋胀痛、乏力、纳差、舌苔薄白、脉弦;证候要素与中药使用之间的关联紧密,可通过靶药改善核心证候要素。结论:CHB肝郁脾虚证的治疗与疏肝、柔肝、健脾有密切的关系,清热解毒药、滋养肝肾药、活血化瘀药的配伍使用贯穿于治疗中,可明显改善病证。Objective:To analyze the use of traditional Chinese medicine in the treatment of chronic hepatitis B(CHB)patients with liver stagnation,spleen deficiency syndrome,and to explore the compatibility of traditional Chinese medicine,the relationship between traditional Chinese medicine and symptoms.Methods:Based on data mining technology,the medical records of CHB patients hospitalized from March 2018 to December 2019 were sorted and summarized according to the inclusion,exclusion criteria.Use data mining methods to analyze the frequency,correlation of prescription traditionalChinese medicines togehter with syndrome elements,and explore the relationship between Chinese medicines and syndrome elements.Results:A total of 35 traditional Chinese medicine prescriptions were included in the study.The main analysis results are as follows:the frequency of use of Radix Paeoniae Alba was 85.7%;the core traditional Chinese medicine prescriptions include Radix Paeoniae Alba,Pericarpium Citri Reticulatae,Radix Bupleur,Carapax Trionycis,Fructus Aurantii,Poria,and Fructus Setariae Germinatus;confidence level The highest two and three combinations of traditional Chinese medicine were Radix Paeoniae Alba-Fructus Aurantii,Radix Paeoniae Alba-Carapax Trionycis,Radix Bupleur;the core syndrome elements of CHB patients with liver depression and spleen deficiency syndrome were flank pain,fatigue,anorexia,tongue coating thin white,pulse string;the relationship between the syndrome elements and the use of traditional Chinese medicine was close,and the core syndrome elements can be improved through target traditional Chinese medicine.Conclusion:The treatment of CHB with liver stagnation and spleen deficiency syndrome is closely related to evacuating the liver,softening the liver,and strengthening the spleen.The compatibility of traditional Chinese medicine for clearing away heat and detoxify cation,nourishing the liver and kidney,promoting blood circulation and removing blood stasis can be used throughout the treatment,which can significantly
