摘  要:目的:观察活血通痹汤联合富血小板血浆治疗膝骨性关节炎(KOA)的临床疗效及对关节液中白介素-1β(IL-1β)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)的影响。方法:选取2018年7月至2020年6月收治的54例膝骨性关节炎患者,给予活血通痹汤口服联合自体富血小板血浆关节腔内注射(PRP)治疗,分别在治疗前、治疗后6周,采用数字评价量表评分(NRS)进行疼痛分析、膝关节损伤与骨关节炎评分(KOOS)进行膝关节疗效评估,参考《中药新药临床研究指导原则》制定中医症状评分,并观察患者关节液中IL-1β、IL-6、TNF-α水平变化。结果:54例患者治疗后NRS评分、KOOS评分、中医症状评分、炎症因子水平均较治疗前明显改善,差异有统计学意义(P<0.05)。结论:活血通痹汤联合富血小板血浆对膝骨性关节炎疗效显著,能降低关节液中炎症因子的表达,是治疗膝骨性关节炎的有效方法。Objective:To observe the efficacy of Huoxue Tongbi decoction combined with platelet rich plasma(PRP) on knee osteoarthritis and its effects on interleukin-1β(IL-1β),interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α).Methods:54 KOA patients during July 2018 to June 2020 were administrated with Huoxue tongbi decoction combined with platelet rich plasma.Before and 6 weeks after treatment,respectively,the pain was analyzed by digital rating scale(NRS);knee joint injury and osteoarthritis score(KOOS) was used to evaluate the efficacy of knee joint;TCM symptom score was made according to the guiding principles of clinical research of new Chinese medicine,and the levels of IL-1β,IL-6,TNF-α in patients’ joint fluid were observed.Results:54 patients in NRS,KOOS,TCM symptom score and inflammatory factor were significantly improved than that before treatment(P<0.05),and it had statistical significance.Conclusion:Huoxue Tongbi decoction combined with PRP has satisfied efficacy on the treatment of knee osteoarthritis and effectively reduce the levels of inflammatory factors in synovia,which is an effective treatment for KOA.
