个性化护理模式在糖尿病肾病血液透析患者中的应用分析

摘　　要：目的研究糖尿病肾病血液透析治疗患者接受个性化护理干预的临床价值。方法选择2017年12月至2019年12月在我院进行血液透析治疗的68例糖尿病肾病患者,通过随机分组的方式分成对照组和观察组,平均每组34例。对照组实施常规血液透析护理;观察组实施个性化护理干预。比较两组患者对护理服务满意度、透析期间低血糖发生情况、护理干预前后透析充分性相关指标、血糖水平、心理状态、生活质量评分、血糖复常和住院总时间。结果观察组患者对护理服务满意度达到94.12%,高于对照组的79.41%,有统计学意义(P<0.05);观察组仅有1例透析期间低血糖事件发生,少于对照组的6例,组间比较有统计学意义(P<0.05);观察组护理干预后透析充分性相关指标、血糖水平、心理状态、生活质量评分的改善幅度大于对照组,有统计学意义(P<0.05);观察组血糖复常和住院总时间短于对照组,有统计学意义(P<0.05)。结论糖尿病肾病血液透析治疗患者接受个性化护理干预,能够帮助保持良好心理状态,有效控制血糖水平,减少透析期间低血糖事件的发生,提高透析充分性,改善生活质量,缩短住院时间,显著提升患者对护理服务的满意度。Objective To study the clinical value of individualized nursing intervention in patients with diabetic nephropathy undergoing hemodialysis.Methods Sixty-eight patients with diabetic nephropathy who underwent hemodialysis in our hospital from December 2017 to December 2019 were randomly divided into control group and observation group,with an average of 34 cases in each group.The control group received routine hemodialysis nursing.The observation group implemented personalized nursing intervention.The nursing service satisfaction,complications,dialysis adequacy related indexes,immune function related indexes,blood glucose level,psychological state,quality of life score,blood glucose normalization and total length of hospital stay were compared between the two groups.Results The satisfaction with nursing service in the observation group was 94.12%,higher than 79.41%in the control group(P<0.05);There was only 1 case of hypoglycemia during dialysis in the observation group,which was less than 6 cases in the control group,and the comparison between the groups was statistically significant(P<0.05).The improvement of dialysis adequacy-related indicators,blood sugar level,psychological state,and quality of life scores in the observation group after nursing intervention was greater than that in the control group,with statistical significance(P<0.05).The normalization of blood glucose and the total length of hospital stay in the observation group were significantly shorter than those in the control group(P<0.05).Conclusion Individualized nursing intervention in patients with diabetic nephropathy and hemodialysis can help maintain a good psychological state,effectively control blood sugar levels,reduce related complications,improve dialysis adequacy,improve quality of life and immune function,shorten hospitalization time,and improve patients'satisfaction with nursing services.
