探析瘘管旷置术配合切开挂线疗法用于复杂性肛瘘治疗的临床效果及对复发率的影响

摘　　要：目的:探讨瘘管旷置术配合切开挂线疗法用于复杂性肛瘘治疗的临床效果及对复发率影响。方法:选取2019年11月~2020年11月就诊的60例复杂性肛瘘患者,按就诊单双日分成A组和B组各30例,均予以两组切开挂线疗法治疗,B组添加瘘管旷置术治疗,并对比手术效果及疾病复发率。结果:在手术时长对比上两组不具对比价值(P>0.05)。在临床疗效对比上A组为80.00%,B组为96.67%;在疾病复发率对比上A组为23.33%,B组为3.33%;在创口愈合时间对比上A组为(25.34±2.79)d, B组为(20.71±1.68)d;在住院时间对比上A组为(17.84±1.37)d, B组为(14.38±0.91)d;在术后疼痛评分、睡眠质量评分、创伤面分泌物评分对比上,B组均低于A组,以上数据对比组间均有统计学意义(P<0.05)。结论:瘘管旷置术联合切开挂线疗法治疗复杂性肛瘘疗效确切,可减少患者术后疼痛及创伤面分泌物,并提升睡眠质量,促进创口愈合加快康复速度,并显著减少术后疾病复发率,具有推广价值。Objective:To investigate the clinical effect of fistula absenteeism combined with incision and thread-drawing therapy in the treatment of complex anal fistula and its effect on the recurrence rate.Methods:A total of 60 patients with complex anal fistula in a hospital from November 2019 to November 2020 were divided into group A and group B,with 30 cases in each group.Both groups were treated with incision and thread-drawing therapy,based on this,group B was treated with fistula exclusion,and the surgical effect and disease recurrence rate were compared.Results:The two groups had no comparative value in comparing the length of operation(P>0.05).In comparison of clinical efficacy,it was 80.00%in group A and 96.67%in group B;In comparison of disease recurrence rate,it was 23.33%in group A and 3.33%in group B;In comparison of wound healing time,it was(25.34±2.79)d in group A and(20.71±1.68)d in group B;The length of hospital stay was(17.84±1.37)days in group A and(14.38±0.91)days in group B;In comparison of postoperative pain score,sleep quality score,and traumatic surface secretion score,group B was lower than group A;The difference in above data between the two groups was statistically significant(P<0.05).Conclusion:Fistula exclusion combined with incision and thread-drawing therapy is effective in the treatment of complex anal fistula,which can reduce postoperative pain and traumatic surface secretions,improve sleep quality,promote wound healing,speed up recovery,and significantly reduce postoperative disease recurrence rate,and has promotion value.
