丙种球蛋白联合阿司匹林在川崎病合并冠脉扩张患儿治疗中的效果观察

摘　　要：目的:探讨丙种球蛋白联合阿司匹林在川崎病(KD)合并冠脉扩张(CAE)患儿治疗中的效果。方法:回顾性收集2019年2月~2020年7月某院收治的89例KD合并CAE患儿的临床资料,将采用阿司匹林治疗的42例患儿纳入对照组,将采用丙种球蛋白联合阿司匹林治疗的47例患儿纳入观察组。比较两组临床症状消失时间、治疗前及治疗5d后免疫功能指标[CD4+、CD8+、CD4+/CD8+、血清免疫球蛋白G(IgG)],观察两组不良反应发生情况。结果:治疗后,较对照组相比,观察组发热、黏膜充血、手足肿大、淋巴结肿大、皮疹症状消失时间均较短,差异有统计学意义(P<0.05);治疗后,两组CD4+、CD4+/CD8+水平均较治疗前降低,两组CD8+、IgG水平均较治疗前升高,且观察组上述指标改善情况优于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率为8.51%,对照组不良反应发生率为4.76%,组间比较,差异无统计学意义(P>0.05)。结论:KD合并CAE患儿采用丙种球蛋白联合阿司匹林治疗,能够有效缩短临床症状消失时间,改善其免疫功能水平,且不会增加不良反应发生率,安全性较高。Objective:To investigate the therapeutic effect of gamma globulin combined with aspirin in Kawasaki disease(KD)patients complicated with coronary dilatation(CAE).Methods:Clinical data of 89 KD children with CAE admitted to a hospital from February 2019 to July 2020 were collected retrospectively.Forty-two children treated with aspirin were included in the control group,and 47 children treated with gamma globulin combined with aspirin were included in the observation group.The disappearance time of clinical symptoms,immune function indexes[CD4+,CD8+,CD4+/CD8+,serum immunoglobulin G(IgG)]before and 5 days after treatment were compared between the two groups,and the incidence of adverse reactions in the two groups was observed.Results:After treatment,compared with the control group,the disappearance time of symptoms of fever,mucosal congestion,hand-foot enlargement,lymph node enlargement and rash was shorter in the observation group,and the difference was statistically significant(P<0.05).After treatment,the levels of CD4+and CD4+/CD8+in both groups were lower than those before treatment,and the levels of CD8+and IgG in both groups were higher than those before treatment.Moreover,the improvement of the above indicators in the observation group was better than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions was 8.51%in the observation group and 4.76%in the control group,and the difference was not statistically significant(P>0.05).Conclusion:Gamma globulin combined with aspirin can effectively shorten the time of disappearance of clinical symptoms and improve the level of immune function in children with KD complicated with CAE,without increasing the incidence of adverse reactions and with high safety.
