真实世界中银花泌炎灵片治疗泌尿系结石的临床用药特点

摘　　要：目的:分析真实世界中银花泌炎灵片治疗泌尿系结石的临床应用特点及联合用药情况。方法:本研究采用关联规则数据挖掘方法对16家全国三级甲等医院信息系统中采集的电子医疗数据进行回顾性分析。结果:纳入559例患者男性居多(65.65%),45~65岁中老年人发病率最高(48.75%),单次口服剂量多在4片(63.07%),疗程8~14 d为主(42.22%),临床常合并泌尿系梗阻、前列腺增生、肾功能不全等疾病。银花泌炎灵联合用药有326种西药和113种中药,其中联用频次最高的单味西药为青霉素(60.64%),中成药为尿石通丸(31.66%);与2种西药3种西药分别联用频次最高中均有聚乙二醇电解质+青霉素(规则支持度45.3%),与2种成药3种中成药分别联用频次最高中均有尿石通丸+舒血宁注射液(规则支持度13.95%)。发现银花泌炎灵片在治疗泌尿系结石时临床常与抗生素、通淋排石剂、活血化瘀剂等联合应用,有助于增强利尿排石、减少结石复发率并预防术后并发症。结论:本研究为银花泌炎灵片在真实世界中的规范用药提供了思路与参考,同时在临床治疗过程中应严格遵守指南及药品说明书的要求,以期达到最佳的临床疗效和安全性保证。Objective:To analyze the clinical application characteristics and combined medication of Yinhua Miyanling Tablets in the treatment of urinary calculi in real world.Methods:In this study,we retrospectively analyzed the electronic medical data collected from 16 large-scale tertiary hospital information system(HIS)by using association rule data mining method.Results:Among the 559 patients included,the majority were male(65.65%),and the incidence was highest in the elderly aged 45-65(48.75%).The single oral dose was mostly 4 tablets(63.07%),and the course of treatment was 8-14 days(42.22%),clinically often complicated with urinary tract obstruction,prostate hyperplasia,renal insufficiency and other diseases.Clinically,there are 326 kinds of western medicines and 113 kinds of Chinese medicines used in combination with Yinhua Miyanling tablets.Among them,the single western medicine with the highest frequency of combination is penicillin(60.64%),the Chinese patent medicine is Niaoshitong piu(31.66%);and 2 kinds of western medicines.,3 kinds of western medicines have the highest frequency of combined use of polyethylene glycol electrolyte+penicillin(regular support 45.3%),and 2 Chinese patent medicines,3 kinds of Chinese patent medicines have the highest frequency of combined use of Niaoshitong pills+Shuxuening injection(regular support 13.95%).It is result that Yinhua Miyanling tablets are often used in combination with antibiotics,Tonglin Paishi,Huoxue Huayu,etc.In the treatment of urinary calculi,which can help to enhance diuresis and elimination,reduce the recurrence rate of stones and prevent postoperative complications disease.Conclusion:This study provides ideas and references for the standardized medication of Yinhua Miyanling tablets in real world.At the same time,in the course of clinical treatment,the requirements of the guidelines and drug inserts should be strictly followed in order to achieve the best clinical efficacy and safety guarantee.
