陈瑞雪应用温胆汤治疗复发性流产的临床经验

摘　　要：温胆汤为祛湿化痰的重要代表方剂,临床应用广泛,多用于心悸、胸闷、呕吐、恶心、眩晕、头痛、不寐等痰湿中阻或痰热扰神的内科疾病。根据“异病同治”理论,温胆汤亦可用于许多妇科疾病的治疗。复发性流产是妇科常见病及多发病,且病因复杂,中医药治疗不明原因的复发性流产具有独特优势。陈瑞雪教授在治疗痰湿中阻、气血不足型复发性流产时擅用温胆汤加减,辨病与辨证相结合,重视调理脾胃、畅达情志、分期治疗及内外同治,取得了显著的临床疗效,提高了复发性流产患者再妊娠成功率。本文从病因病机、治法、方剂、治疗特点等方面阐述其治疗复发性流产的经验,并结合典型病案展示其具体诊断及用药思路、辨证论治及加减用药原则,为广大同仁提供一种治疗思路。Wendan decoction is an important representative prescription of clearing damp and reducing phlegm.It is widely used in clinical practice,it is mainly used in internal diseases such as palpitation,chest tightness,vomiting,nausea,dizziness,headache,insomnia and so on.According to the theory of"treating different diseases with the same treatment",Wendan decoction can also be used for the treatment of many gynecological diseases.Recurrent spontaneous abortion is a common gynecological disease with a high incidence,the cause of RSA is complex,Chinese medicine has a unique advantage in the treatment of RSA for unknown reasons.Professor CHEN Rui-xue has made good use of Wendan Decoction in the treatment of RSA caused by phlegm dampness and insufficiency of Qi and blood.She makes a good combination of disease identification and dialectics,emphasized recuperating the spleen and stomach,Adjusting emotions,treating by stages,treating internal and external.She has achieved remarkable clinical efficacy,significantly improved the success rate of re-pregnancy in patients with RSA.This paper expounds Professor CHEN Ruixue's experience in the treatment of RSA from the aspects of etiology,treatment,prescription,treatment characteristics,and combines the typical disease cases,shows her specific diagnosis and medication ideas,syndrome differentiation,treatment and addition and subtraction principles,to provide a kind of treatment idea for the colleagues.
