摘  要:评价不同运动方式对非酒精脂肪肝患者脂代谢干预效果。方法:数据库选择为中国知网(CNKI)、万方、维普、Web of Science、PubMed、Cochran library、Medline、Embase等数据库,纳入文献的发表日期为2010年1月-2021年3月,由两名研究人员收集运动干预非酒精脂肪肝病的随机对照试验(RCT),按PRISMA声明制定纳入排除标准并提取数据,进行质量评价。采用Stata16.0软件对最终纳入的项研究进行meta分析。结果:纳入13项随机对照研究,Meta分析结果表明,运动干预可显著降低NAFLD患者体内血清总胆固醇水平(SMD=-0.24,95%CI-0.40~-0.08,P<0.01);甘油三酯水平(SMD=-0.30,95%CI-0.39~-0.20,P<0.01);低密度脂蛋白水平(SMD=-0.22,95%CI-0.32~-0.12,P<0.01);根据Begg′s秩相关检验和Egger′s直线回归检验结果表明,纳入研究发表偏倚水平较低。结论:运动强度为最大心率的65%-75%,每周三次,每次30-60min持续12周以上可显著降低NAFLD患者脂代谢指标。To evaluate the intervention effect of different exercise modalities on patients with non-alcoholic fatty liver.METHODS:The databases were selected as China Knowledge Network(CNKI),Wanfang,Wipu,Web of Science,PubMed,Cochran library,Medline,and Embase,and the publication dates of the included literature were from January,2010 to March,2021,and two researchers collected exercise interventions for nonalcoholic fatty liver disease randomized controlled trials(RCTs),with inclusion and exclusion criteria developed and data extracted for quality evaluation according to the PRISMA statement.A meta-analysis of the final included studies was performed using Stata 16.0 software.RESULTS:Thirteen randomized controlled studies were included,and the results of the meta-analysis showed that exercise intervention significantly reduced total serum cholesterol levels in patients with NAFLD(SMD=-0.24,95%CI-0.40 to-0.08,P<0.01);triglyceride levels(SMD=-0.30,95%CI-0.39~-0.20,P<0.01);LDL levels(SMD=-0.22,95%CI-0.32~-0.12,P<0.01);according to Begg′s rank correlation test and Egger′s linear regression test,the level of publication bias in the included studies was low.CONCLUSION:Exercise intensity of 65%-75%of maximum heart rate for 30-60 min three times a week for more than 12 weeks significantly reduces lipid metabolism indexes in patients with NAFLD.
