绝经后妇女IUD取出情况及取出困难相关危险因素分析

摘　　要：目的分析绝经后妇女宫内节育器(IUD)取出情况及取出困难的相关危险因素。方法纳入2019年1月至2020年7月我院收治的要求取出IUD的1 540例绝经后妇女,均行IUD取出术,记录IUD取出情况。根据IUD是否取出困难将绝经妇女分为对照组和困难组。比较两组的临床资料,使用单因素分析及二元Logistic回归分析探讨绝经后妇女IUD取出困难的影响因素。结果 1 540例绝经后妇女中,IUD取出困难率为24.81%。IUD取出困难者IUD的宫内状态中,嵌顿占比最高(61.26%)。绝经时间、子宫后位、IUD放置时间、剖宫产、IUD置入医院级别均为绝经后妇女IUD取出困难的危险因素(P<0.05)。结论绝经后IUD取出困难者多为IUD嵌顿,其发生受个体情况、医源等多类型因素的影响,在对绝经后妇女进行IUD取出术时,应积极了解其病史、IUD置入时间等情况,明确其高危因素,以及时调整IUD取出方案,提高IUD取出术的安全性。Objective To analyze the intrauterine device(IUD) removal and risk factors related to difficulty in IUD removal in postmenopausal women.Methods A total of 1 540 postmenopausal women requiring IUD removal admitted in our hospital from January 2019 to July 2020 were included,all underwent IUD removal surgery and the condition of IUD removal were recorded.According to whether it was difficult to remove the IUD,postmenopausal women were divided into control group and difficult group.The clinical data of the two groups were compared,and the influencing factors of IUD removal difficulty in postmenopausal women were explored by univariate analysis and binary Logistic regression analysis.Results Among 1 540 postmenopausal women,the difficulty rate of IUD removal was 24.81%.In the intrauterine state of IUD with difficulty in IUD retrieval,incarceration accounted for the highest proportion(61.26%).Menopausal time,uterus retroposition,IUD placement time,cesarean section,level of hospital carrying out IUD implantation were all risk factors for the difficulty of IUD removal in postmenopausal women(P<0.05).Conclusion IUD incarceration is the most common condition of difficulty in removing IUD for postmenopausal women.Its occurrence is affected by various factors such as individual conditions and iatrogenic sources.When performing IUD removal for postmenopausal women,their medical history,IUD insertion time and so on should be actively understand,so as to clarify the high-risk factors,and adjust the IUD removal plan in time to improve the safety of IUD removal surgery.
