硝苯地平联合拉贝洛尔对妊高征患者血管功能、凝血功能及妊娠结局的影响

摘　　要：目的观察硝苯地平联合拉贝洛尔对妊娠高血压综合征(简称妊高征)患者血管功能、凝血功能及妊娠结局的影响。方法随机选择2019年1月至2020年12月来我院诊治的150例妊高征患者为研究对象,按随机数字表法将其分为对照组和观察组,各75例。对照组采用硝苯地平治疗,观察组采用硝苯地平联合拉贝洛尔治疗。比较两组的血压控制效果、血管功能指标、凝血功能指标及母婴不良妊娠结局。结果治疗后,两组SBP、DBP均降低,且观察组低于对照组(P<0.05)。治疗后,两组AC均升高,Eρ均降低,且观察组优于对照组(P<0.05)。治疗后,两组ET-1、sVCAM-1水平均降低,NO水平均升高,且观察组优于对照组(P<0.05)。治疗后,两组APTT、TT均延长,FIB水平均降低,且观察组优于对照组(P<0.05)。观察组新生儿及产妇不良妊娠结局总发生率均低于对照组(P<0.05)。结论硝苯地平联合拉贝洛尔治疗妊高征可有效控制血压,改善血管弹性、内皮功能及凝血功能,优化妊娠结局,值得临床应用。Objective To observe the effects of nifedipine combined with labetalol on vascular function,coagulation function and pregnancy outcome in patients with pregnancy-induced hypertension.Methods A total of 150 patients with pregnancy-induced hypertension treated in our hospital from January 2019 to December 2020 were randomly selected as the research objects.The patients were divided into control group and observation group by random number table method,with 75 cases in each group.The control group was treated with nifedipine,and the observation group was treated with nifedipine combined with labetalol.The blood pressure control effect,vascular function indexes,coagulation function indexes and maternal and infant adverse pregnancy outcome were compared between the two groups.Results After treatment,SBP and DBP in the two groups decreased,and those in the observation group were lower than the control group(P<0.05).After treatment,AC increased and Eρ decreased in the both groups,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of ET-1 and sVCAM-1 decreased and the level of NO increased in the two groups,and those in the observation group were better than the control group(P<0.05).After treatment,APTT and TT prolonged and FIB level decreased in the two groups,and those in the observation group were better than the control group(P<0.05).The total incidences of neonatal and maternal adverse pregnancy outcomes in the observation group were lower than those in the control group(P<0.05).Conclusion Nifedipine combined with labetalol in the treatment of pregnancy-induced hypertension can effectively control blood pressure,improve vascular elasticity,endothelial function and coagulation function,and optimize pregnancy outcome,which is worthy of clinical application.
