赣南地区幽门螺旋杆菌与2型糖尿病和代谢综合征相关性研究

摘　　要：目的:探讨赣南地区幽门螺旋杆菌(Hp)与2型糖尿病(T2DM)和代谢综合征(MS)的相关性。方法:采用多阶段随机抽样的横断面研究方法,抽取2020年11月—2021年4月赣南地区常住人口3200例为研究对象,比较Hp阳性组与Hp阴性组的T2DM和MS发病率,T2DM和MS患者Hp阳性组与Hp阴性组的临床相关指标。结果:Hp阳性组T2DM和MS发病率明显高于Hp阴性组,T2DM患者Hp阳性组空腹血糖(FPG)、糖化血红蛋白(HbA1c)、胰岛素抵抗指数(HOMA-IR)明显高于Hp阴性组,MS患者Hp阳性组体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、FPG、HbA1c、HOMA-IR、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)明显高于Hp阴性组,高密度脂蛋白(HDL-C)明显低于Hp阴性组,差异均有统计学意义(P<0.05)。结论:赣南地区Hp感染与T2DM和MS的发生密切相关,加强Hp的筛查及根治对这两种疾病具有重要意义。Objective:To investigate the relationship between Helicobacter pylori(Hp)and type 2 diabetes(T2DM)and metabolic syndrome(MS)in southern Jiangxi.Methods:A multi-stage random sampling cross-sectional study method was adopted.From November 2020 to April 2021,3200 permanent residents in southern Jiangxi were selected as the research objects.The Hp infection and the incidence of T2DM and MS were collected and compared between the Hp positive group and The incidence of T2DM and MS in the Hp-negative group,the clinically relevant indicators of the Hp-positive group and Hp-negative group of T2DM patients,and the Hp-positive group and Hp-negative group of MS patients.Results:The incidence of T2DM and MS in the Hp-positive group was significantly higher than that of the Hp-negative group(P<0.05),the fasting blood glucose(FPG),glycosylated hemoglobin(HbA1c)and insulin resistance index(HOMA-IR)of the Hp-positive group of T2DM patients were significantly higher than those of Hp Negative group(P<0.05).MS patients with Hp positive group body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),FPG,HbA1c,HOMA-IR,total cholesterol(TC),triglycerides(TG)and low-density lipoprotein(LDL-C)were significantly higher than the Hp-negative group,and high-density lipoprotein(HDL-C)was significantly lower than the Hp-negative group(P<0.05).Conclusion:Hp infection is closely related to the occurrence of T2DM and MS in southern Jiangxi.It is of great significance to strengthen Hp screening and radical treatment for these two diseases.
