中药熏洗联合关节松动训练治疗创伤性肘关节僵硬31例临床观察

摘　　要：目的:探究中药熏洗联合关节松动训练对创伤性肘关节僵硬患者的临床疗效。方法:选取创伤性肘关节僵硬患者62例,采用随机数字表法分为对照组和观察组各31例。对照组进行关节松动训练治疗;观察组在对照组的基础上进行中药熏洗,观察两组中医临床疗效,对比治疗前及治疗4周后两组的肘关节肿胀程度、肘关节主动关节活动度(AROM)、Mayo肘关节功能评分(MEPS)。结果:治疗后,两组中医证候积分、肘关节周径均降低(P<0.05),且对照组高于观察组(P<0.05);治疗后,两组MEPS评分均升高(P<0.05),且对照组低于观察组(P<0.05);治疗后,两组AROM屈曲、伸展角度、总活动范围均增加(P<0.05),且对照组小于观察组(P<0.05)。结论:中药熏洗联合关节松动训练可提高创伤性肘关节僵硬患者中医临床疗效,改善临床症状评分,缓解关节肿胀,有效改善肘关节活动范围,提高肘关节功能。Objective To explore the clinical effect of traditional Chinese medicine fumigation combined with joint mobilization training on patients with traumatic elbow stiffness.Methods62 patients with traumatic elbow stiffness were randomly divided into control group(31 cases)and observation group(31 cases).The patients in the control group were treated with joint mobilization training,while the patients in the observation group were treated with traditional Chinese medicine fumigation on the basis of the control group.The clinical efficacy of traditional Chinese medicine in the two groups was observed.The degree of elbow swelling,active range of motion(AROM)and Mayo elbow function score(MEPs)of the two groups were compared before and 4 weeks after treatment.Results after treatment,the TCM symptom score and elbow circumference of the two groups were decreased(P<0.05),and the control group was higher than the observation group(P<0.05);After treatment,the MEPs scores of the two groups were increased(P<0.05),and the control group was lower than the observation group(P<0.05);After treatment,AROM flexion,extension angle and total range of motion of the two groups were increased(P<0.05),and the control group was less than the observation group(P<0.05).ConclusionChinese herbal fumigation combined with joint mobilization training can improve the clinical efficacy of traditional Chinese medicine in patients with traumatic elbow stiffness,improve the score of clinical symptoms,relieve joint swelling,effectively improve the range of motion of elbow joint and improve the function of elbow joint.
