摘 要:目的分析血清维生素D、白细胞介素-8(interleukin-8,IL-8)、白细胞介素-10(interleukin-10,IL-10)、转化生长因子-β1(transforminggrowthfactor-β1,TGF-β1)水平,探讨其与早产儿支气管肺发育不良(bronchopulmonary Dysplasia,BPD)的关系。方法选取海口市妇幼保健院收治的166例早产儿,分为BPD组和NBPD组,并把BPD组内以病情严重程度分为轻、中、重度三组,分析患儿血清1,25-(OH)_(2)D_(3)、IL-8、IL-10、TGF-β1水平。结果两组患儿血清1,25-(OH)_(2)D_(3)、IL-8、IL-10、TGF-β1胎龄及体重水平差异具有统计学意义(均P<0.05),其中BDP组患儿血清血清1,25-(OH)_(2)D_(3)和IL-10均低于NBPD组(P<0.05),IL-8和TGF-β1高于NBPD组(P<0.05),BPD组患儿胎龄及体重均小于NBPD组。BPD患儿组内血清1,25-(OH)_(2)D_(3)、IL-8、IL-10、TGF-β1水平比较差异具有统计学意义(均P<0.05),与轻度组比较,中度组与重度组BPD患儿血清1,25-(OH)_(2)D_(3)和IL-10水平均降低(P<0.05),IL-8和TGF-β1均升高(均P<0.05),且中度组1,25-(OH)_(2)D_(3)和IL-10高于重度组(均P<0.05),中度组IL-8和TGF-β1小于重度组(均P<0.05)。1,25-(OH)_(2)D_(3)和IL-10水平与BPD病情严重程度呈负相关(均P<0.05),IL-8和TGF-β1与BPD病情严重程度呈正相关(均P<0.05)。结论血清维生素D、IL-8、IL-10、TGF-β1水平与早产儿支气管肺发育不良密切相关,有望成为临床BPD诊治的有效指标。Objective To analyze the serum vitamin D,interleukin-8(IL-8),interleukin-10(IL-10),transforming growth factor-β1(TGF-β1)levels,and explore its relationship with bronchopulmonary dysplasia(BPD)in premature in‐fants.Methods One hundred and sixty-six preterm infants admitted to Haikou Maternal and Child Health Hospital were selected and divided into BPD and NBPD groups,and divided into mild,moderate and severe groups according to the severity of the disease within the BPD group,and analyzed the serum 1,25-(OH)_(2)D_(3),IL-8,IL-10 and TGF-β1 levels of the children.Results The differences in serum levels of 1,25-(OH)_(2)D_(3),IL-8,IL-10 and TGF-β1 in the BDP group were statistically significant(t=8.494,17.363,8.615,8.722,P<0.05).The serum levels of 1,25-(OH)_(2)D_(3) and IL-10 in the BDP group were lower than those in the NBPD group(P<0.05),and IL-8 and TGF-β1 were higher than those in the NB‐PD group(P<0.05).The differences in serum levels of 1,25-(OH)_(2)D_(3),IL-8,IL-10,and TGF-β1 in the groups of children with BPD were statistically significant(F=16.469,345.743,23.931,25.800,P<0.05).Among them,compared with the mild group,the levels of serum 1,25-(OH)_(2)D_(3) and IL-10 in children with BPD in the moderate group and severe group decreased(P<0.05),and both IL-8 and TGFβ1 increased(P<0.05),and the 1,25-(OH)_(2)D_(3) and IL-10 in the moderate group were higher than those in the severe group(P<0.05),and the IL-8 and TGF-β1 in the moderate group were lower than those in the severe group(P<0.05).The levels of 1,25-(OH)_(2)D_(3) and IL-10 were negatively correlated with the sever‐ity of BPD(r=-0.567,-0.635,P<0.05),and IL-8 and TGF-β1 were positively correlated with the severity of BPD(r=0.937,0.651,P<0.05).Conclusion Serum levels of vitamin D,IL-8,IL-10,and TGF-β1 are closely related to broncho‐pulmonary dysplasia in premature infants,and are expected to become effective indicators for clinical diagnosis and treatment of BPD.
