微创关节镜下医用钢丝与Orthocord缝线内固定治疗胫骨髁间棘骨折效果观察

摘　　要：目的比较微创关节镜下医用钢丝与Orthocord缝线内固定治疗胫骨髁间棘骨折的效果。方法选择2016年6月—2018年5月收治的胫骨髁间棘骨折123例,根据手术方法分为研究组63例和对照组60例。研究组给予微创关节镜下Orthocord缝线内固定治疗,对照组给予微创关节镜下医用钢丝内固定治疗。比较2组手术情况、膝关节功能评分、Lachman试验结果、临床疗效及并发症发生情况。结果2组手术时间、术中出血量比较差异无统计学意义(P>0.05)。研究组骨折愈合时间短于对照组(P<0.01)。研究组膝关节功能各项评分及总分高于对照组(P<0.05,P<0.01)。研究组Lachman试验阳性率低于对照组,总有效率高于对照组(P<0.05)。2组并发症发生率比较差异无统计学意义(P>0.05)。结论Orthocord缝线内固定治疗胫骨髁间棘骨折的效果优于微创关节镜下医用钢丝内固定,临床可根据患者情况选择治疗方案。Objective To compare the efficacy between medical wire and Orthocord suture for minimally invasive arthroscopic internal fixation in treatment of patients with fracture of intercondylar spine of tibia.Methods A total of 123 patients with fracture of intercondylar spine of tibial treated between June 2016 and May 2018 were selected and divided into research group(n=63)and control group(n=60)according to surgical methods.Research group was treated with Orthocord suture internal fixation under minimally invasive arthroscopy,while control group was treated with medical steel wire internal fixation under minimally invasive arthroscopy.Operative condition,knee joint function score,Lachman test result,clinical efficacy and incidence rate of complications were compared between two groups.Results There were no significant differences in operative duration and intraoperative volume of bleeding between two groups(P>0.05).The fracture healing time in research group was significantly shorter than that in control group(P<0.01).Scores of knee joint function and the total score in research group were significantly higher than those in control group(P<0.05,P<0.01).In research group,positive rate of Lachman test was significantly lower than that in control group,while the total effective rate was significantly higher than that in control group(P<0.05).There was no significant difference in incidence rate of complications between two groups(P>0.05).Conclusion The efficacy of Orthocord suture in treatment of patients with fracture of intercondylar spine of tibia is better than that by medical wire in minimally invasive arthroscopic internal fixation,and therefore clinical treatment plan can be flexible based on the patient's condition.
