自拟三黄苦参汤坐浴联合龙胆泻肝汤内服治疗肛门湿疹效果观察

摘　　要：目的探讨自拟三黄苦参汤坐浴联合龙胆泻肝汤内服治疗肛门湿疹的效果。方法选取2019年10月—2021年2月就诊的肛门湿疹79例,按治疗方法分为观察组40例和对照组39例。对照组给予龙胆泻肝汤内服治疗,观察组在对照组基础上联合自拟三黄苦参汤坐浴治疗。比较2组临床疗效,治疗前后症状积分、肛门功能及血清炎性因子水平,并进行安全性评价。结果观察组总有效率高于对照组(P<0.05)。2组治疗后各症状积分、总积分及肿瘤坏死因子-α、白细胞介素-2(IL-2)、IL-4和IL-5均较治疗前降低,且观察组低于对照组(P<0.05)。2组治疗后肛管静息压、肛管最大收缩压和直肠静息压均较治疗前升高,且观察组高于对照组(P<0.05)。2组不良反应发生率比较差异无统计学意义(P>0.05)。结论自拟三黄苦参汤坐浴联合龙胆泻肝汤内服治疗肛门湿疹可改善临床症状和肛门功能,提高临床疗效,安全性好。Objective To explore the efficacy of sitz bath with self-made Sanhuang Kushen Decoction combined with oral administration of Longdan Xiegan Decoction in treatment of patients with anal eczema.Methods A total of 79 patients with anal eczema treated between October 2019 and February 2021 were selected and divided into observation group(n=40)and control group(n=39)according to different therapeutic methods.Control group was treated with Longdan Xiegan Decoction by oral administration,while observation group was treated with Sanhuang Kushen Decoction on the basis of treatment for control group.Clinical efficacy,symptom score,anal function and serum levels of inflammatory factors before and after treatment were compared between two groups,and the safety was also evaluated.Results After treatment,the total effective rate in observation group was significantly higher than that in control group(P<0.05).After treatment,symptom scores,the total scores,levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-2,IL-4 and IL-5 were significantly lower than those before treatment in two groups,and the above indexes in observation group were significantly lower than those in control group(P<0.05).After treatment,anal resting pressure,maximum systolic pressure of anal canal and rectal resting pressure were significantly higher than those before treatment in two groups,and the above indexes in observation group were significantly higher than those in control group(P<0.05).There was no significant difference in incidence rate of adverse reactions between two groups(P>0.05).Conclusion Sitz bath with self-made Sanhuang Kushen Decoction combined with oral administration of Longdan Xiegan Decoction may significantly improve clinical symptoms and anal function,effectively inhibit inflammatory reactions and improve clinical efficacy with good safety in treatment of patients with anal eczema.
