摘 要:根据《中国心血管病报告2014》的数据显示,我国高血压患者已接近3亿人,高血压作为最常见的心血管疾病,患病人数庞大,需要更好的医疗技术与手段管理血压,尽可能减少心血管疾病风险。动态血压监测作为高血压诊断技术发展史上的重大创新,能够准确而全面地反映血压整体情况,在临床应用的30余年中,动态血压监测能够准确发现“隐蔽性高血压”,包括单纯夜间高血压,并很好地避免“白大衣性”高血压,24 h动态血压监测不仅可以了解血压变化趋势,还包括血压负荷、负荷指数、血压分布模式、血压下降率、低谷/峰比以及一昼夜血压总体变异情况,越来越多的研究表明动态血压监测还可应用于临床其他病因及疾病的判断与诊断中。本文就动态血压监测最新临床应用进行综述,并总结临床应用中存在的问题,以便未来将动态血压监测更好应用于临床诊断中。According to the data of China Cardiovascular Disease report 2014,there are nearly 300 million patients with hypertension in China.Hypertension,as the most common cardiovascular disease,has a large number of people,so better medical techniques and means are needed to manage blood pressure and reduce the risk of cardiovascular disease as much as possible.As an important innovation in the history of hypertension diagnosis technology,ambulatory blood pressure monitoring(ABPM)can accurately and comprehensively reflect the overall situation of blood pressure.In the past 30 years of clinical application,ambulatory blood pressure monitoring can accurately detect"hidden hypertension",including simple night hypertension,and avoid"white coat hypertension"very well.24-hour ambulatory blood pressure monitoring can not only understand the changing trend of blood pressure,but also include blood pressure load.Load index,blood pressure distribution model,blood pressure decline rate,trough/peak ratio and the overall variation of one day and night blood pressure,more and more studies have shown that ambulatory blood pressure monitoring can also be used in the judgment and diagnosis of other clinical causes and diseases.In this paper,the latest clinical application of ambulatory blood pressure monitoring is reviewed,and the existing problems in clinical application are summarized in order to better apply ambulatory blood pressure monitoring to clinical diagnosis in the future.
