摘　　要：目的:探讨经阴道四维宫腔输卵管超声造影(TVS 4D-HyCoSy)联合宫腔水造影(SIS)对不孕症的诊断价值。方法:选取60例不孕症患者为研究对象,患者均接受TVS 4D-HyCoSy及SIS检查,且经腹腔镜及宫腔镜最终确诊。比较TVS 4D-HyCoSy、TVS 4D-HyCoSy联合SIS诊断输卵管通畅性、子宫病变的效能及受检期间不良反应发生情况。结果:与金标准相比,TVS 4D-HyCoSy、TVS 4D-HyCoSy联合SIS诊断输卵管通畅性的灵敏度、特异度、准确度分别为85.71%/91.43%、79.27%/93.90%、81.20%/93.16%,二者特异度与准确度比较,差异有统计学意义(P<0.05)。TVS 4D-HyCoSy联合SIS诊断子宫病变的灵敏度(75.00%vs.91.67%)与TVS 4D-HyCoSy单独诊断比较,差异无统计学意义(P>0.05);特异度(83.33%vs.95.83%)、准确度(81.67%vs.95.00%)高于TVS 4D-HyCoSy(P<0.05)。结论:TVS 4D-HyCoSy联合SIS是诊断不孕症安全、高效的手段。Objective:To investigate the diagnostic value of transvaginal four-dimensional hysterosalpingo-contrast sonography(TVS 4D-HyCoSy)combined with salineinfusionsonography(SIS)in infertility.Methods:60 female infertility patients were selected.All patients underwent TVS 4D-HyCoSy and SIS examination,and were diagnosed by laparoscopy and hysteroscopy.The efficacy of TVS 4D-HyCoSy,TVS 4D-HyCoSy combined with SIS in the diagnosis of tubal patency and uterine lesions was compared,and the adverse reactions during the examination were recorded.Results:Compared with the gold standard,the sensitivity,specificity and accuracy of TVS 4D-HyCoSy,TVS 4D-HyCoSy combined with SIS in the diagnosis of tubal patency were 85.71%/91.43%,79.27%/93.90%and 81.20%/93.16%,respectively,and the differences were statistically significant(P<0.05).The sensitivity of TVS 4D-HyCoSy combined with SIS in the diagnosis of uterine lesions was not significantly different from that of TVs 4D hycosy alone(75.00%vs.91.67%,P>0.05),the specificity(83.33%vs.95.83%)and accuracy(81.67%vs.95.00%),which were significantly higher than those of TVS 4D-HyCoSy(P<0.05).Conclusion:For female infertility,TVS 4D-HyCoSy combined with SIS is a safe and efficient diagnostic method.
