摘　　要：目的 研究母牛分枝杆菌疫苗联合生长激素改善获得性免疫缺陷综合征(AIDS)患者免疫重建不良的效果。方法 选取患者135例作为研究对象,随机分为4组,A组肌肉注射母牛分枝杆菌疫苗,B组肌肉注射重组人生长激素,C组采用安慰剂治疗,D组肌肉注射母牛分枝杆菌疫苗+重组人生长激素。疗程为48周,治疗后分别检测各组患者体内静脉血纯真T细胞、调节性T细胞(Treg)、总CD4^(+)T细胞、Th1细胞、Th2细胞、白细胞介素-7(IL-7)细胞因子水平变化。结果 A组、B组和D组中,AIDS患者体内总CD4^(+)T细胞、Th1细胞、纯真T细胞水平增加,IL-7细胞因子、Treg、Th2细胞水平下降,差异均有统计学意义(P<0.05),C组无变化。A组与B组Th1细胞下降幅度比较,差异有统计学意义(P<0.05);A组与D组各项指标比较差异均无统计学意义(P>0.05)。结论 母牛分枝杆菌疫苗可改善AIDS患者免疫重建不良状态,疗效与重组人生长激素相当,但费用更低。Objective To research the effect of mycobacterium bovis vaccine combined with growth hormone on immune reconstitution in patients with acquired immunodeficiency syndrome(AIDS).Methods A total of 135 patients were selected as the research object,and divided into four groups.The group A was injected intramuscularly with mycobacterium vaccae vaccine.The group B was injected intramuscularly with recombinant human growth hormone.The group C was treated with placebo.The group D was injected intramuscularly with mycobacterium vaccae vaccine plus recombinant human growth hormone.The course of treatment was 48 weeks.After treatment,the cytokines levels of pure T cells,regulatory T cells(Treg),total CD4^(+)T cells,Th1 cells,Th2 cells and interleukin-7(IL-7)in venous blood of patients in each group were detected.Results In the group A,B and D,the levels of total CD4^(+)T cells,Th1 cells and pure T cells in AIDS patients increased,while the levels of IL-7 cytokines,Treg cells and Th2 cells decreased(P<0.05).There was no change in the group C.There was a significant difference in the decreasing range of Th1 cells between group A and group B(P<0.05).There was no statistical difference in each index between the group A and group D(P>0.05).Conclusion Mycobacterium vaccae vaccine can improve the poor immune reconstitution of AIDS patients.Its curative effect is equivalent to that of recombinant human growth hormone,but its cost is lower.
