2018年-2020年甘肃省天水市发热呼吸道症候群病原体的研究分析

摘　　要：目的了解2018年—2020年甘肃省天水地区发热呼吸道疾病患者病原体构成,分析其流行特征。方法首先进行细菌培养,然后用多重实时荧光PCR方法进行病原体核酸检测。结果通过对407份发热呼吸道疾病患者样本检测,共检出22种病原体,阳性267例,总阳性率为65.6%(267/407),前9种病原体阳性232例,阳性率为57.0%(232/407)。阳性率较高的前4位依次为肺炎链球菌PRSP(53.2%)、流感嗜血杆菌HiB(25.8%)、流感病毒Flu(13.5%)、呼吸道合胞病毒RSV(11.2%)。前9种病原体感染情况:每年10月—次年3月的检出率较高,12月检出阳性病例数最高,存在明显的季节性变化。男性感染占63.8%,女性感染占36.2%;≤3岁组检出率最高。混合感染者有117例,占50.4%(117/232),以PRSP和HiB的混合感染最为常见。结论2018年—2020年甘肃省天水地区最主要的病原体构成为PRSP、HiB、Flu、RSV 4种,其他病原体也占有较大的比例。Objective To understand the pathogen composition of patients with febrile respiratory disease in Tianshui area from 2018 to 2020 and analyze its epidemiological characteristics.Methods Bacterial culture was carried out firstly,and then the nucleic acid of the pathogen was detected by multiplex real-time PCR.Results A total of 267 cases were positive for 22 pathogens.The total positive rate was 65.6%(267407),and the positive rate was 57.0%(232/407)for the first nine pathogens.The highest positive rate was Streptococcus pneumoniae PRSP(53.2%),Haemophilus influenzae HiB(25.8%),influenza flu(13.5%)and human respiratory syncytial virus RSV(11.2%).The infection rate of the first 9 pathogens was higher from October to the next March every year,the positive cases were the highest in December,and there were obvious seasonal changes,the infection rate was 63.8%in male and 36.2%in female.The detection rate was the highest in the group≤3 years old.Mixed infection was found in 117 cases(50.4%,117/232).Mixed infection of PRSP and HIB was the most common.Conclusion From 2018 to 2020,the main pathogens in Tianshui area were PRSP,HibB,Flu and RSV,and other pathogens also accounted for a large proportion.
