摘　　要：目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)与早期动脉粥样硬化和10年心血管疾病(CVD)风险的关系。方法选取OSAS患者250例,根据睡眠呼吸暂停低通气指数AHI的高低将OSAS患者分为3组:轻度OSAS组(n=98)、中度OSAS组(n=84)和重度OSAS组(n=68);对照组选取同期的健康体检者160例;用Framingham风险评分模型评估10年CVD风险,用肱踝脉搏波传导速度(baPWV)反映早期动脉粥样硬化。结果OSAS组baPWV水平和Framingham风险评分显著高于对照组(P<0.05);随着OSAS病情严重程度的增加,baPWV水平[(1205.68±18.51)cm/svs(1413.44±21.49)cm/svs(1645.87±24.49)cm/s,P<0.05]和CVD中高风险者所占的比例(17.34%vs 26.19%vs 47.05%,P<0.05)均逐渐升高;Logistic回归分析提示OSAS是早期动脉粥样硬化和10年CVD风险增高的独立危险因素。结论OSAS与早期动脉粥样硬化及10年CVD风险密切相关。Objective To investigate the association of obstructive sleep apnea syndrome(OSAS)with early atherosclerosis and 10-year risk of cardiovascular disease(CVD).Methods A total of 250 patients with OSAS were enrolled and divided into three groups:the mild OSAS group(n=98),moderate OSAS group(n=84)and severe OSAS group(n=68)according to the sleep apnea hypopnea index(AHI).A total of 160 healthy subjects were selected as the control group.The 10-year CVD risk was assessed by Framingham Risk Score Model and early atherosclerosis was assessed by brachial and ankle pulse wave conduction velocity(baPWV).Biochemical indexes were determined in all participants.Results baPWV level and Framingham risk score were significantly higher in the OSAS group than the control group(P<0.05).With the increase of severity of OSAS,baPWV level[(1205.68±18.51)cm/s vs(1413.44±21.49)cm/s vs(1645.87±24.49)cm/s,P<0.05]and the proportion of high risk CVD patients(17.34%vs 26.19%vs 47.05%,P<0.05)were gradually increased.Logistic regression analysis showed that OSAS was an independent risk factor for early atherosclerosis and 10-year CVD risk.Conclusion OSAS is closely associated with early atherosclerosis and 10-year CVD risk.
