改良复尔凯管空肠造瘘置管在胸腹腔镜微创食管癌切除术中的应用体会

摘　　要：目的探讨改良复尔凯管空肠造瘘置管在胸腹腔镜微创食管癌切除术中的应用的安全性、可行性及临床效果。方法回顾性分析2012年3月至2019年7月于南京医科大学附属肿瘤医院和南京医科大学附属逸夫医院进行的112例胸腹腔镜食管癌切除手术患者资料,比较改良后的腹腔镜下复尔凯管空肠造瘘置管术和腹腔镜下分裂式空肠造瘘置管术两组间空肠造瘘手术时间、造瘘管相关并发症、进流质时间、造瘘管留置时间及平均住院日的差异。结果与分裂式空肠造瘘置管术比较,复尔凯管空肠造瘘置管术手术时间明显缩短(P<0.05);造瘘管相关并发症的发生率明显减少(P<0.001);术后开始进流质时间明显缩短(P=0.031);带管时间无明显差异(P=0.832);平均住院时间缩短(P=0.018)。结论腹腔镜下复尔凯管空肠造瘘置管术具有操作简便、易于掌握,且安全、可行、具有良好的近期疗效,值得在食管癌微创术中推广。Objective To investigate the safety,feasibility and clinical effect of modified laparoscopic flocare jejunostomy in thoracoscopic and laparoscopic minimally invasive resection of esophageal cancer.Methods The data of 112 patients who underwent thoracoscopic and laparoscopic esophagectomy in the Affiliated Tumor Hospital of Nanjing Medical University and Sir Run Run Hospital of Nanjing Medical University from March 2012 to July 2019 were analyzed retrospectively.The operation time of jejunostomy between the two groups was compared Differences in fistula related complications,inflow time,fistula indwelling time and average hospital stay.Results Compared with the group of split jejunostomy tube placement,the operation time of flocare jejunostomy tube placement group was significantly shorter(P<0.05);the incidence of tube-related complications was significantly reduced(P<0.001);The time of liquid diet intake was significantly shortened(P=0.031);there was no significant difference in the indwelling time of jejunal nutrition tube(P=0.832);the average length of hospitalization was decreased(P=0.018).Conclusion Laparoscopic flocare jejunostomy tube placement is simple,easy to master,safe,feasible,and has a good short-term effect,and it is worthy of promotion in minimally invasive surgery for esophageal cancer.
