升督槐榆汤联合痔瘘祛毒熏洗剂保守治疗痔疮的临床观察

摘　　要：【目的】观察升督槐榆汤联合痔瘘祛毒熏洗剂保守治疗痔疮的临床疗效。【方法】将120例痔疮(混合痔)患者随机分为治疗组和对照组,每组各60例。治疗组患者给予升督槐榆汤联合痔瘘祛毒熏洗剂治疗,对照组患者单纯给予痔瘘祛毒熏洗剂治疗,疗程为7 d。观察2组患者治疗前后的便血、痔核脱出、肛门肿痛、痔核黏膜充血糜烂等症状体征评分的变化情况,并评价2组患者的中医证候疗效。【结果】(1)中医证候疗效方面:治疗7 d后,治疗组的愈显率和总有效率分别为78.3%(47/60)和98.3%(59/60),对照组分别为33.3%(20/60)和80.0%(48/60),组间比较,治疗组的愈显率和总有效率均明显优于对照组,差异均有统计学意义(P<0.05或P<0.01)。(2)症状体征评分方面:治疗后,2组患者的便血、痔核脱出、肛门肿痛、痔核黏膜充血糜烂等各项症状体征评分均较治疗前降低(P<0.05),且治疗组的上述各项症状体征评分均明显低于对照组,差异均有统计学意义(P<0.05)。【结论】升督槐榆汤联合痔瘘祛毒熏洗剂保守治疗痔疮疗效显著,其疗效优于单纯痔瘘祛毒熏洗剂熏洗治疗。Objective To observe the clinical efficacy of Shengdu Huaiyu Decoction combined with Zhilou Qudu Fumigant for the conservative treatment of hemorrhoids. Methods A total of 120 patients with hemorrhoids(mixed hemorrhoids)were randomly divided into the treatment group and the control group, 60 cases in each group.Patients in the treatment group were treated with Shengdu Huaiyu Decoction orally combined with fumigation and washing with Zhilou Qudu Fumigant,while patients in the control group were given fumigation and washing with Zhilou Qudu Fumigant alone,the course of treatment for the two groups lasting for 7 days. The changes of the symptom and sign scores of bloody stool,prolapse of hemorrhoids,perianal swelling and pain,congestion and erosion of hemorrhoid mucosa in the two groups were observed before and after treatment. After treatment, the TCM syndrome efficacy of the two groups was evaluated. Results(1)TCM syndrome efficacy: after 7 days of treatment, the cured and markedly effective rate and the total effective rate were 78.3%(47/60)and 98.3%(59/60)respectively in the treatment group, and were 33.3%(20/60)and 80.0%(48/60)respectively in the control group. The intergroup comparison showed that the cured and markedly effective rate and the total effective rate in the treatment group were superior to those in the control group, the differences being statistically significant(P<0.05 or P<0.01).(2)After treatment,symptom and sign scores of bloody stool,prolapse of hemorrhoids,perianal swelling and pain,congestion and erosion of hemorrhoid mucosa in the two groups were lower than those before treatment(P<0.05),and the scores in the treatment group after treatment were significantly lower than those in the control group, the differences being statistically significant(P<0.05). Conclusion Oral use of Shengdu Huaiyu Decoction combined with fumigation and washing with Zhilou Qudu Fumigant has significant therapeutic effect on hemorrhoids,and its curative effect is superior to that of fumigation and washing with Zhi
