万晓刚加减运用四逆散的临证经验

摘　　要：四逆散为《伤寒论》专为气郁而设立的名方,具有疏肝理脾、透邪解郁之功。基于对四逆散调肝理脾、疏调气机功效的认识,万晓刚教授以四逆散为基础方,化裁运用于各种气血失调病证的治疗,尤以治疗甲状腺疾病、消化系统疾病和妇科疾病的效果突出。治疗气血痰饮搏结之甲状腺疾病,分期辨证并辨明病理因素,对症处理,常配合散结、化痰、清热、化瘀、养阴药;治疗气机升降失常、痰饮壅滞之消化系统疾病,重在疏理肝胆,调畅气机,常配合补气、逐痰、降逆、通络药;治疗气血失和、痰瘀壅塞之妇科疾病,以调和气血、化痰散结、化瘀消癥为法,常配合培元、补血、调肝、化痰、化瘀药,疗效显著。Sini Powder is a famous formula indicated for qi stagnation recorded in Treatise on Exogenous Febrile Disease, which has the functions of soothing liver and regulating spleen, expelling pathogens and relieving depression. For Sini Powder has the actions of regulating liver and spleen,soothing and regulating qi movement,Professor WAN Xiao-Gang prescribed the recipes derived from Sini Powder to treat various syndromes of qi and blood disharmony, especially in the treatment of thyroid disease, digestive system disease and gynecological disease. For the treatment of thyroid diseases caused by the accumulation of qi, blood and phlegm-retention,staging treatment and identification of the pathological factors are needed, and symptomatic treatment can be achieved by the combination of herbs for dissipating mass, resolving phlegm, clearing heat, removing blood stasis and nourishing yin. For the treatment of digestive system disease with abnormal qi movement and accumulation of phlegm-retention, soothing liver and regulating gallbladder and regulating qi movement are stressed, and the herbs for replenishing qi, eliminating phlegm, checking upward adverse flow of qi and unblocking collaterals can be added for modified treatment. For the treatment of gynecological disease with disharmony of qi and blood and blockage of phlegm and blood stasis,the methods of harmonizing qi and blood,resolving phlegm to dissipate mass,removing stasis and eliminating abdominal mass can be used,often assisted by the combination of herbs for cultivating the vital essence, enriching blood, harmonizing liver, resolving phlegm and removing blood stasis to achieve a significant effect.
