施光其辨治皮肌炎并发肺间质纤维化经验

摘　　要：施光其教授认为皮肌炎并发肺间质纤维化的病机为正虚为本,邪实为标;正虚为肺脾气虚,邪实为风、寒、湿、热乘虚侵袭人体,六气化火,内舍肺脏。治疗皮肌炎肺间质纤维化可参考肺痿论治,分为风伤皮毛、热伤血脉二型,在祛风方(主要由荆芥、防风、地肤子等组成)的基础上加减治疗,分别以祛风散寒、化痰散瘀及祛风止咳、清热化痰凉血为法,辨证论治,可取良效。Professor SHI Guang-Qi believes that the pathogenesis of dermatomyositis complicated with pulmonary interstitial fibrosis is characterized by deficiency of healthy qi in the origin,manifesting as qi deficiency of both lung and spleen,while excess of the pathogenic qi in the superficiality,manifesting as the pathogens of wind,cold,dampness and heat invading the body as the body was at the weak state and then transforming into fire and retaining in the lung. The treatment of dermatomyositis complicated with pulmonary interstitial fibrosis can follow the therapeutic principle of pulmonary atrophy,which is differentiated into wind attacking skin and hair type and heat impairing blood vessel type. On the basis of Qufeng Formula(the wind-dispelling prescription mainly composed of Herba Schizonepetae,Radix Saposhnikoviae,Fructus Kochiae,etc.),the modified treatment with the herbs for dispelling wind and dispersing cold,resolving phlegm to dissipate stasis and herbs for dispelling wind to ease cough,clearing heat and resolving phlegm and cooling blood based on syndrome differentiation can achieve positive clinical effect for the treatment of dermatomyositis complicated with pulmonary interstitial fibrosis.
