摘　　要：目的分析大连市友谊医院2020年1月—2020年12月鲍曼不动杆菌的临床分布以及耐药性,以期为临床医师对鲍曼不动杆菌感染患者的临床诊断和用药提供理论依据。方法收集大连市友谊医院2020年1月—2020年12月鲍曼不动杆菌共153株,应用法国Biomerieux公司Vitek-2分析仪进行细菌鉴定和体外药敏实验,用Whonet5.6分析药敏数据,SPSS19.0软件对重症医学科和非重症医学科的耐药率进行比较。结果153株鲍曼不动杆菌主要分布在痰液(56.21%)和肺泡灌洗液(29.41%),血液(0.65%)和胸腔积液(1.31%)检出率较低,病区主要分布在ICU(54.90%)、新生儿重症监护室(Neonatal intensive care unit,NICU)(12.42%)和老年病(9.15%)等科室。鲍曼不动杆菌对哌拉西林/他唑巴坦(79.1%)和环丙沙星(85.7%)耐药率较高,对替加环素和多黏菌素耐药性最低(0%),ICU细菌耐药性远高于非ICU细菌。结论大连市友谊医院鲍曼不动杆菌的耐药性不容乐观,要通过严格控制抗生素的合理使用,加强临床科室的消毒,做好临床患者相关的防护工作,严格控制处方权等手段对降低鲍曼不动杆菌的耐药性。Objective This paper analyzes the clinical distribution and drug resistance of Acinetobacter baumannii from January 2020 to December 2020 at Dalian Friendship Hospital with a view to providing theoretical basis for clinicians’clinical diagnosis and medication of patients infected with Acinetobacter baumannii.Methods A total of 153 strains of Acinetobacter baumannii were collected from January 2020 to December 2020 in Friendship Hospital of Dalian.The bacteria were identified by Vitek-2 analyzer of Biomerieux Company of France,and the drug sensitivity data were analyzed by Whonet5.6.SPSS19.0 software was used to compare the rates of drug resistance between critical care and non-critical care medicine.Results Clinically,153 Acinetobacter baumannii detected in different specimens were mainly distributed in sputum(56.21%)and alveoli irrigation fluids(29.41%),Blood(0.65%)and thoracic fluid(1.31%)detection rate is low.The main source of Acinetobacter baumannii were from ICU(54.90%),NICU(12.42%)and geriatrics department(9.15%).The resistant rate of bacteria to Piperacillin Tazobactam(79.1%)and Ciprofloxacin(85.7%)was higher.The resistance to tigecycline and Polymyxin was the lowest(0%),and the bacterial resistance in ICU was much higher than that in non-ICU.Conclusion The drug resistance of Acinetobacter baumannii in Dalian Friendship Hospital is not optimistic.It is necessary to strictly control the use of antibiotics,strengthen the disinfection of clinical departments,do a good job in the protection of clinical patients,and strictly control prescription rights to reduce the resistance of Acinetobacter baumannii.
