摘　　要：目的评估中老年股骨粗隆间骨折(IFF)患者创伤后成长水平,重点分析IFF患者创伤后成长水平的影响因素。方法回顾性分析安徽省阜阳市第二人民医院2017年1月至2021年2月收治的105例中老年IFF患者的临床资料,采用创伤后成长评定量表(PTGI)评估其创伤后成长水平,将PTGI评分<70分的患者纳入创伤后成长水平低下组(39例),将PTGI评分≥70分的患者纳入创伤后成长水平正常组(66例)。收集并比较两组基线资料,分析中老年IFF患者创伤后成长水平的影响因素。结果创伤后成长水平低下组社会支持评定量表(SSRS)评分、遵医行为评分、心理弹性量表(CD-RISC)评分低于创伤后成长水平正常组,差异有统计学意义(P<0.05)。多因素分析结果显示,高SSRS评分、高遵医行为评分、高CD-RISC评分是中老年IFF患者创伤后成长水平低下的保护因素(OR<1,P<0.05)。结论中老年IFF患者创伤后成长水平可能受社会支持、遵医行为、心理弹性等因素影响。Objective To evaluate the level of posttraumatic growth in middle-aged and elderly patients with intertrochanteric fracture of femur(IFF),and to analyze the influencing factors of posttraumatic growth in IFF patients.Methods Data of 105 middle-aged and elderly IFF patients admitted to Fuyang Second People’s Hospital,Anhui Province from January 2017 to February 2021 were retrospectively collected and analyzed.The level of posttraumatic growth was assessed by the posttraumatic growth inventory(PTGI).Patients with PTGI score<70 points were included in the low posttraumatic growth group(39 cases),and patients with PTGI score≥70 points were included in the normal posttraumatic growth group(66 cases).The baseline data of two groups were collected and compared to analyze the influencing factors of posttraumatic growth in middle-aged and elderly IFF patients.Results The scores of social support rating scale(SSRS),compliance behavior score,and Connor-Davidson resilience scale(CD-RISC)in the low posttraumatic growth group were lower than those in the normal posttraumatic growth group,and the differences were statistically significant(P<0.05).Multivariate analysis showed that high SSRS score,high compliance behavior score,and high CD-RISC score were protective factors for low level of posttraumatic growth in middle-aged and elderly IFF patients(OR<1,P<0.05).Conclusion The posttraumatic growth level of middle-aged and elderly IFF patients may be affected by social support,medical compliance behavior,psychological resilience,and other factors.
