2020年河北省沧州市沧县基本公共卫生服务项目实施效果评估,以高血压为例

摘　　要：目的了解沧州市沧县高血压患者通过国家基本公共卫生服务项目中高血压患者健康管理后,血压控制情况及其影响因素,为提高高血压患者控制达标率提供参考依据。方法选取国家基本公共卫生服务项目管理平台上沧州市沧县2020年管理的65周岁以上高血压患者为研究对象,了解患者高血压控制情况并分析影响血压控制因素。结果通过项目实施沧州市沧县居民高血压管理血压控制达标率为91.13%;男性血压控制达标率为90.22%,女性血压控制达标率为91.88%,差异有统计学意义(P<0.05);65~74岁和>74岁辖区居民的血压控制达标率分别为91.31%和90.72%,差异有统计学意义(P<0.05)。结论体质指数、吸烟、饮酒、摄盐情况、遵医行为、服药依从性、随访次数、管理年限均对高血压患者血压控制有影响(P<0.05)。今后工作中应通过高血压患者戒烟、限酒、降低食盐摄入量提高高血压患者的辖区管理水平,最终提高血压控制率。Objective To understand the blood pressure control status and influencing factors of hypertensive patients in Cangxian County of Cangzhou City after passing the health management of hypertensive patients in the national basic public health service project,so as to provide a reference for improving the control rate of hypertension patients.Methods Selected hypertensive patients over 65 years old who were managed by Cangxian County,Cangzhou City in 2020 on the national basic public health service project management platform as the research objects.Understood the patient's hypertension control and analyzed the factors that affect blood pressure control.Results Through the implementation of the project,the compliance rate of blood pressure control for residents in Cangxian County,Cangzhou City was 91.13%.The blood pressure control compliance rate of men was 90.22%,and that of women was 91.88%,the difference was statistically significant(P<0.05).The compliance rate of blood pressure control among residents aged 65-74 years old and>74 years old was 91.31%and 90.72%,respectively,and the difference was statistically significant(P<0.05).Conclusion Body mass index,smoking,drinking,salt intake,compliance behavior,medication compliance,follow-up times,and management years all have an impact on blood pressure control in patients with hypertension(P<0.05).In the future,the management level of the jurisdiction of hypertensive patients should be improved by quitting smoking,restricting alcohol,and reducing salt intake for hypertensive patients,and ultimately improving the control rate of blood pressure.
