太仓市医疗安全不良事件报告障碍影响因素分析

摘　　要：目的分析影响医护人员报告医疗安全不良事件的障碍因素,为改进医疗安全不良事件报告系统提供参考。方法于2019年9月11—12日,向江苏省太仓市123名医护人员发放电子调查问卷,调查问卷分为四个部分,分别为一般性资料、医疗安全不良事件上报情况、遇到医疗安全不良事件后的行动以及影响医疗安全不良事件上报的因素。使用主成分因子分析法对影响因素进行维度分类,使用卡方检验对不良事件报告行为进行分析,使用方差分析(F检验)对医疗机构之间的维度因素进行分析。结果该地区63.41%的医护人员近一年没有报告过医疗安全不良事件。医护人员遇到医疗安全不良事件后,更倾向选择“向科主任/护士长汇报”。影响综合医院和专科医院不良事件报告的维度因素主要是“社交态度”,而乡镇卫生院则为“制度管理”。“保护同事或下属”是制约该地区医疗安全不良事件报告的最重要因素。结论社交因素和制度管理的薄弱是制约不良事件上报的主要维度因素,特别是基层医疗机构不良事件报告系统亟待建立和完善。Objective To investigate the barriers that affect medical staff to report medical safety adverse events,and provide a reference for improving the medical safety adverse event reporting system.Methods From September 11,2019 to September 12,2019,an electronic questionnaire was distributed to 123 medical staff in Taicang city,Jiang-su province.The questionnaire was divided into four parts:general information,reporting of adverse medical safety events,actions after encountering adverse medical safety events,and factors affecting the reporting of adverse medical safety events.Principal component factor analysis was used to classify the influencing factors in dimensions,the chis-quare test was used to analyze the adverse event reporting behavior,and the analysis of variance(F test)was used to analyze the dimensional factors between medical institutions.Results 63.41%of the medical staff in this area had not reported adverse medical safety events in the past year.When medical staff encounter adverse medical safety events,they were more inclined to choose"report to the head of the section/head nurse."The main dimensional factor affecting adverse event reports in general hospitals and specialty hospitals was"social attitude",while township hospitals were"system management"."Protecting colleagues or subordinates"was the most important factor restricting the reporting of adverse events in medical safety in this region.Conclusion Social considerations and weak system management are the main dimensional factors restricting the reporting of adverse events.In particular,the adverse event reporting system of primary medical institutions needs to be established and improved.
