喻昌论治中风病学术思想探讨及现代临床应用

摘　　要：通过整理分析《医门法律》《尚论篇》《尚论后篇》《寓意草》及相关文献,探讨喻昌治疗中风病经验和学术思想。总结其治疗中风病理论如下:1)主要病机为阳虚邪乘,本虚标实;火、气、痰、郁等体质偏颇对中风病具有易感性;2)以全面的四诊合参为依据进行脏腑辨证,尤重脉诊;3)首提“驱风之中,兼填空窍”的治则;以雨治风,思路新颖;4)明辨病程,动态治疗,依据病邪深浅不同,顺势采用不同治法;5)中风病贵在预防和病后调护,强调节欲。现代医者在中风病的病因病机、诊断、治疗和预防调护方面借鉴并发挥了喻昌的学术思想,直到今天对于中风病诊疗仍具有指导意义和实用价值。By sorting out and analyzing Principle and Prohibition for Medical Profession,Shanglunpian,Shanglunhoupian and Yuyicao,along with related literature,discussing the experience and academic ideas of Yu Chang to treat stroke disease.The academic ideas of Yu Chang to treat stroke disease are presented as below:1)The pathogenesis is deficiency of yang and hitting of pathogen,which is the syndrome of deficiency in origin and excess in superficiality.Body constitution,such as fire-heat intensity,disturbance of qi activity,phlegm and qi depression will make the susceptibility raise.2)Using comprehensive analysis of data gained by Four Examinations to make syndrome differentiation of zangfu viscera,and particularly emphasizing pulse diagnosis.3)He first put forward the rule of expelling wind and filling up superficial orifices,which is innovative to advance the theory that wind could be annihilated by rain.4)Differentiating courses of stroke disease and using different therapies according to the course of disease.5)It is important to take precautions against stroke and nursing after acute phase,and continence is also stressed.Modern doctors have been using Yu Chang’s academic thought for reference in etiology,pathogenesis,diagnosis,treatment,prevention and care of stroke.Up to now,it still has guiding significance and practical value for the diagnosis and treatment of stroke.
