摘 要:目的观察亚硒酸钠治疗桥本甲状腺炎(HT)甲减患者的临床疗效,评估对血清TPO-Ab及氧化应激水平的影响,并分析其作用机制。方法将80例HT甲减患者随机分为对照组和研究组,每组各40例,对照组应用左甲状腺素治疗;在对照组治疗的基础上研究组同时应用亚硒酸钠,比较两组临床疗效,检测血清TSH、FT_(3)、FT_(4)、TPO-Ab、TGAb、SOD、MDA、Th17细胞、IL-17、IL-23水平。结果研究组患者总有效率为80.00%,明显高于对照组(P<0.05);经治疗后研究组患者血清中FT_(3)、FT_(4)、TSH、TPO-Ab、TGAb、SOD、MDA均显著优于对照组(P<0.05)。研究组治疗后血Th17细胞、IL-17、IL-23分别为(12.13±0.31)%、17.28±0.40pg/mL和14.06±0.21pg/mL,均明显低于对照组,其差异具有统计学意义(P<0.05)。结论亚硒酸钠应用于HT甲减患者能够显著改善患者甲状腺激素和氧化应激水平,且患者血清TPO-Ab滴度、血Th17细胞、IL-17、IL-23均显著降低。Objective To observe the clinical efficacy of sodium selenite in the treatment of patients with Hashimoto’s thyroiditis(HT),and to evaluate the effect of serum thyroid peroxidase antibody(TPO-Ab)titer and oxidative stress and analyze its underlying mechanism.Methods 80 HT patients with hypothyroidism were randomly divided into the control group and study group,wiht 40 cases in each group.The control group was treated with levothyroxine conventional therapy,while in addition,the study group was also treated with sodium selenite at the same time to compare the clinical efficacy.We detected serum TSH,FT_(3),FT_(4),TPOAb,TGAb,SOD,MDA,Th17 cells,IL-17,and IL-23 levels for the analysis.Results The total effective rate of patients in the study group was 80.00%,which was significantly higher than that in the control group(P<0.05);After treatment,the serum levels of FT_(3),FT_(4),TSH,TPOAb,TGAb,SOD,and MDA in the study group were significantly better than those in the control group(P<0.05).The blood levels of Th17 cells,IL-17 and IL-23 in the study group were(12.13±0.31)%,17.28±0.40pg/mL and 14.06±0.21pg/mL,respectively,lower than the control group,and the difference was statistically significant(P<0.05).Conclusion The use of sodium selenite in patients with HT hypothyroidism can significantly improve thyroid hormone and oxidative stress levels,and patients’serum TPO-Ab titer,blood Th17 cells,IL-17,IL-23 show a downward trend.
