摘　　要：目的:构建糖尿病肾病患者家庭照顾者照顾活动和支持行为的结构方程模型,探讨各因素的作用机制。方法:采用便利抽样方法,选取天津市2所三级甲等医院住院的280例糖尿病肾病患者家庭照顾者进行调查。采用巴氏指数评定量表、社会支持评定量表、一般自我效能感量表、Zarit护理负担量表、糖尿病照顾者活动和支持量表收集资料。运用Amos 21.0软件构建结构方程模型,对假设模型进行检验和评价。结果:被照顾者自理能力、家庭照顾者社会支持、自我效能对照顾活动和支持行为存在直接的影响效应,照顾负担对照顾活动和支持行为存在直接的影响效应,自理能力和社会支持可通过照顾负担的中介作用对照顾活动和支持行为产生间接的影响。结论:照顾负担在照顾活动和支持行为的作用机制中是不容忽视的,在自理能力和社会支持因素对照顾活动和支持行为的影响过程中起着中介的作用。Objective:To explore the mechanism of each factor and construct a structural equation model of caring activities and supportive behaviors of family caregivers of diabetic kidney disease patients.Methods:A convenience sampling method was used to select 280 family caregivers for patients with diabetic kidney disease admitted to two Grade A hospitals in Tianjin.Data was collected using structured questionnaires which included Barthel Index Scale,Social Support Rating Scale,General Self-efficacy Scale,Zarit Caregiver Burden Interview Scale,and Diabetes Caregiver Activity and Support Scale.Amos 21.0 software was used to construct the structural equation model,and the hypothesis model was tested and evaluated.Results:Self-care ability,family caregivers of social support and self-efficacy had a direct effect on caring activities and supportive behaviors,while care burden had a direct effect on caring activities and supportive behaviors,and self-care ability and social support had an indirect effect on caring activities and supportive behaviors through the mediating effect of care burden.Conclusion:The burden of care cannot be ignored in the mechanism of caring activities and supportive behaviors,and it plays an intermediary role in the process of the influence of self-care ability and social support factors on caring activities and supportive behaviors.
