复方青黛胶囊(丸)治疗寻常型银屑病的成本-效用分析

摘　　要：目的对复方青黛胶囊(丸)治疗寻常型银屑病进行成本-效用分析。方法检索建库至2020年11月4日国内外中英文数据库,收集复方青黛胶囊(丸)治疗银屑病的临床试验研究文献。基于文献对主要结局指标进行Meta分析,并构建7个健康状态的马尔科夫模型模拟分析复方青黛胶囊(丸)联合常规治疗与常规治疗两种方案治疗寻常型银屑病的长期(10年)成本和健康产出。结果Meta分析结果显示,复方青黛胶囊(丸)联合常规治疗组与常规治疗组的有效率分别为92.5%和73.8%,差异有统计学意义(P<0.0001)。成本-效用分析结果显示,在模型模拟的长期(10年)治疗中,复方青黛胶囊(丸)联合常规治疗组平均治疗成本为54372.45元,获得的质量调整生命年(QALYs)为2.40;常规治疗组平均治疗成本为47225.59元,QALYs为2.36;复方青黛胶囊(丸)联合常规治疗组相比常规治疗组的增量成本-效果比(ICER)为174136.62元/QALY,单因素敏感性分析和概率敏感性分析显示基础分析结果稳健。结论在中国目前的药品价格水平和诊疗情境下,与常规治疗相比,复方青黛胶囊(丸)联合常规方案治疗寻常型银屑病具有良好的健康产出和经济性。Objective To analyze the cost Utility of compound Qingdai Capsules(Pills)in the treatment of Psoriasis Vulgaris.Methods The clinical experimental research literature of Compound Qingdai Capsule(Pills)in the treatment of psoriasis were searched from the database of Chinese and English from the establishment of the database to November 4,2020.Based on the literature,the main outcome indicators were meta analyzed,and the Markov model of 7 health status was established to simulate the long-term(10-year)cost and health output of Compound Qingdai Capsule(pill)combined with conventional treatment and conventional treatment in the treatment of psoriasis vulgaris.Results Meta analysis showed that the effective rate of Compound Qingdai Capsule(Pills)combined with conventional treatment group and conventional treatment group was 92.5%and 73.8%,respectively,the difference was statistically significant(P<0.0001).The results of cost utility analysis showed that the average cost of treatment in the combination of Compound Qingdai Capsule(pill)and conventional treatment group was 54372.45 yuan in the long-term(10-year)treatment model simulation,and the quality adjusted life year(QALYs)was 2.40 years;The average cost of treatment in the conventional treatment group was 47225.59 yuan,and the QALYs was 2.36 years.The incremental cost effectiveness ratio(ICER)of the experimental group compared with the control group was 174136.62 yuan/QALY.The single factor sensitivity analysis and probability sensitivity analysis showed the robustness of the basic analysis results.Conclusion In the current drug price level and diagnosis&treatment situation in China,compared with conventional treatment,the combination of compound Qingdai Capsules(Pills)and conventional treatment for psoriasis vulgaris has better health outcome and economy.
