433例肺血栓栓塞症患者血液C-反应蛋白、抗凝血酶和纤维蛋白原及血液学变化特点分析

摘　　要：目的研究肺血栓栓塞症(pulmonary thromboembolism,PTE)患者的C反应蛋白(CRP)、抗凝血酶(AT)和纤维蛋白原(FB)等凝血指标和血液学变化特点及其与PTE发生的关系。方法选取433例经螺旋CT肺动脉造影(computed tomography pulmonary angiogrophy,CTPA)确诊的PTE患者,对患者的临床特点、CRP,AT,FB,血浆D二聚体(D-D)和血小板(PLT)计数以及凝血指标进行综合分析,并探讨其相关的临床意义。结果①PTE总发生率1.60‰(433/270983),60岁以上为高发年龄段。②D-D升高412例(95.15%)。PLT计数正常331例(76.44%),降低53例(12.24%),升高49例(11.31%)。③CRP升高75.99%(288/379)、正常24.01%(91/379)。④AT降低109例(25.17%),FB升高102例(23.56%)。其他凝血指标无特殊异常变化。⑤CRP与PLT,FB呈正相关(r=0.1026,t=2.0118,P<0.05和r=0.0.5360,t=12.2963,P<0.01)。⑥CRP与AT呈负相关(r=-0.2005,t=4.0100,P<0.01)。结论住院患者PTE发生率较高。多数PTE患者高表达CRP和FB,低表达AT,CRP。AT和FB可能与PTE发生有关。Objective To study the characteristics of coagulation indexes and hematological,changes such as C-reactive protein(CRP),antithrombin(AT)and fibrinogen(FB)in patients with pulmonary thromboembolism(PTE)and their relationship with PTE.Methods The clinical characteristics,D-D,PLT counts,CRP,AT,and FB of 433 patients with PTE confirmed by CTPA were analyzed comprehensively,and the clinical significance was discussed.Results①The total incidence of PTE was 1.60‰(433/27083),over 60 years old was the age of high incidence.②D-D elevation was found in 412 cases(95.15%).PLT were normal in 331 cases(76.44%),decreased in 53 cases(12.24%)and increased in 49 cases(11.31%);③CRP was increased in 288 cases(75.99%)and normal in 91 cases(24.01%).④AT was decreased in 109 cases(25.17%)and FB was increased in 102 cases(23.56%).There were no abnormal changes in other coagulation indexes.⑤CRP was positively correlated with PLT and FB(r=0.1026,t=2.0118,P<0.05 and r=0.0.5360,t=12.2963,P<0.01,respectively).⑥CRP was negatively correlated with AT(r=-0.2005,t=4.0100,P<0.01).Conclusion Most patients with PTE had high expression of CRP and FB,and low expression of AT and CRP.AT and FB may be related to the occurrence of PTE.
